There is also an additional Level of Care Cost that is based upon an assessment conducted by our Director of Nursing prior to any move-in, that I have broken down for you below:
The point system we utilize to determine level of care is as follows:
- Level 0: Requires no care/ 0 points = $0 additional cost
- Level 1: 1-9 points = $912.00 additional per month
- Level 2: 10-19 points = $2,037 additional per month
- Level 3: 20-29 points = $2,646 additional per month
- Level 4: 30-39 points = $3,244 additional per month
*Level of care cost caps at a level 4*

Could you provide an overview on the types of professionals on your staff and the training they undergo at your facility?
Pathfinder’s leadership team consists of our Executive Director, Director of Nursing, Sales and Marketing Coordinator, Maintenance Manager, Environmental Services Manager, Business Office Manager, and Activities Director. Our Director of Nursing is on call for care 24/7 and is expected to come to the facility with any care concerns that may occur after hours. Our care team consists of mainly CNAs and Medication Aides, although we do have an LPN that works for us, and the Sales and Marketing Coordinator is also a Registered Nurse. All Pathfinder employees are put through extensive training and can be offered additional training if they are not yet comfortable with their position. We also require all employees to complete monthly Relias courses that provide ongoing continuing education to all Pathfinder employees.
Could you tell me about the measures you have in place to ensure the safety and well-being of your residents?
All residents require a signed plan of care by their Primary Care Physician prior to move so we know exactly how to care for our residents and their individual needs. Our interdisciplinary team reviews all resident care plans quarterly as well as every time there is a change in a resident’s condition. Safety concerns are always discussed with the team and any additional safety measures taken in order to keep a resident safe, will be documented on the resident’s plan of care. All residents are provided a safety pendant that they can push when wanting staff assistance, and all apartments have safety pull cords in each room that also notify the care team of the resident’s need for assistance.
Do you offer any resources or support services to aid residents' transition into the community?
We understand that moving is hard for our residents and we do offer counseling services through our third-party providers for any counseling or grief services that may be required. We also do “Getting to Know You” forms with all residents upon move in that helps all Pathfinder staff get to know the residents likes, hobbies, needs, etc., which helps us as a community ensure.
Do you offer transportation services for residents?
Yes- Appointments are scheduled with our Business Office Manager Tiffany to ensure that there is van availability for transportation and that appointments do not get double booked.
What is the process that takes place if a resident has a complaint or an issue they want addressed?
We have grievance forms set out in several areas of our community for residents and families to voice their concerns. Once a grievance form is complete, the grievance is given to the Executive Director who ensures the grievance is handled at her earliest convenience, with the standard turnaround time being no later than seven business days. At Pathfinder, we look at grievances as a gift, as we cannot fix anything we do not know about!
What is your policy on communication with resident's families? How do you keep them updated about their loved ones' well-being?
Our executive Director and DON share the responsibility of Pathfinders Family Call Program. All families are contacted monthly as well as with any changes in a resident’s condition, medications, or level of care. A resident or their family can always voice concerns with the Executive Director or DON via phone or in person.
Do you all offer short-term or respite stays? How much does this cost?
Pathfinder offers short-term respite stays (Minimum of 2 weeks) up to 30 days at a rate of $200/day.
Are you all a pet-friendly community?
Yes, we do allow pets; however, a resident or a visitor of Pathfinder must provide their pet’s current vaccination records to the Pathfinder Executive Director prior to the pet entering the community.
Do you guys currently have a waitlist?
No- Although there are times we are at full capacity, we are not full at this time. We have two apartments available currently.
Are residents able to get Covid vaccination and flu vaccinations in the community?
Yes, Our DON is able to provide annual flu vaccinations and Covid vaccines in the community.
Facility Overview
Pathfinder is a 39-bed facility that is all on one level. We are a giant square with an enclosed courtyard in the middle. Because Pathfinder is a private pay facility, we have different regulations than the majority of our Assisted Living competitors.
Age in Place
Pathfinder residents are able to “Age in Place”, meaning we don’t make you move out of our community with any decline in your health status. We can accommodate residents with a higher acuity of care, similar to what nursing homes and long-term care facilities offer, but at about half the cost.
Payment Options
We do accept aid and attendance as well as long term care insurance policies if applicable.
Safety Features
All residents are provided a safety pendant upon move in that they utilize when needing staff assistance. There are also pull cords available in the apartment bedrooms, living room areas, and bathrooms.
Apartment Amenities
All apartments have a kitchenette area that comes with a mini fridge and microwave as well as a large private bathroom with a walk-in shower. We also have a brand new jacuzzi whirlpool in our bath house available if residents prefer a bath over a shower.
Activities and Events
We have an extremely robust activity calendar, with several fun upcoming events for our residents. We also host a family night once a month for our residents and their families.
Dining
Meals are served restaurant style in our dining room. If a resident does not like the main dish being served, we have an “Always Offered” menu available with hamburger, hotdog, chef salad, soup, or sandwich substitutions.
Health Services
We utilize third-party providers for home health and hospice services for residents who qualify. We also have a nurse practitioner group, Health At Home Consultants, who round our building weekly servicing residents who have acute illnesses going on that may not be able to get into their primary physician right away.

We service both traditional and memory care residents.
Security
We have a secure coded entryway to ensure safety for both our residents and staff at all times.
Visiting Hours
We have no restrictions on visiting hours at Pathfinder, so friends and family members of our residents can visit any time!
