Care Levels
Level of care charges are determined based on each individual’s need for assistance. Our highly-trained staff can assist with bathing, dressing, toileting/incontinence care, medication administration, diabetic management, oxygen therapy and mobility assistance. We are happy to estimate your loved ones’ care level, although the final care level and related cost cannot be determined until the initial care plan is developed. Individual care plans are reviewed quarterly to ensure that we are providing the necessary assistance for your loved one.

Overview of Staff and Training
All our staff is required to have a minimum of 12 hours of continuous education to maintain a position here at PH. We typically offer training twice a month for our staff to acquire those hours, resulting in 24 opportunities and hours available for extra training each year. Additionally, we provide one-on-one, hands-on training when staff members are hired before they work independently. Everyone must pass a background check, among other requirements, before hiring. Our staff ranges from culinary-trained Dining Room professionals, Housekeeping, maintenance, CNAs, med aides, caregivers, Nurses, Admin staff, to Management.Safety and Well-being Measures
Our Assisted Living (AL) residents receive a call button for emergency use. Memory Care (MC) residents generally engage with activity and nursing staff. Those who are unable or choose not to participate are safely monitored in a common area with staff or checked on frequently. We have 24-hour trained staff, locked exterior doors, secured and private courtyards, and meal attendance is recorded three times a day to ensure residents are eating, drinking, and accounted for. Each resident has a personalized care plan that may include additional safety checks based on their needs.Support Services for Transitioning
We have a Resident Liaison who assists new residents by inviting them to activities, introducing them to staff, other residents, and their table mates, easing the transition into the community and making sure they don’t feel like a fish out of water.Transportation Services
We offer transportation to and from four medical appointments a month. Residents also go on trips with activity staff to destinations such as Walmart, Target, and Hy-Vee, usually once a week. Scheduled outings and scenic rides are also accompanied by our activity staff.Dining Experience
Our dining room offers beautiful views of the Champions golf course, specifically the tee box for hole #7. We provide two meal choices along with sides, desserts, soups, and various beverages. An alternative menu is available for those who prefer different options, which can be ordered at the time of the meal and made to order. Residents enjoy restaurant-style dining with meal choices made at each dining instance.Handling Complaints
Residents can share concerns at any time. We strive to hear them out and find solutions or explain necessary processes. Monthly "Town Hall" meetings empower residents to participate in decision-making, sharing their thoughts, and offering suggestions while interacting with department leaders.Communication with Families
Communication is essential among staff, residents, and their families. We provide updates on changes in condition, falls, injuries, illnesses, and other notable events. Families should feel free to share any concerns noted during visits, and we will address them. Care plans can change, and we ensure they are appropriate for each resident's needs.Short-term or Respite Stays
We generally stay quite full, making it challenging to keep a room available for respite stays without hindering the accommodation of permanent residents. Respite stays are not frequently needed; therefore, they are not a typical offering.Pet-friendly Policy
Pets are welcome, whether living with residents or visiting for therapy. Rabies vaccinations are required for any community-entrant pets, and those living with residents need an assessment. A pet deposit is also required for resident pets.Vaccination Services
We offer a flu and COVID vaccination clinic once a year for residents who wish to receive these services.Aging in Place
We support aging in place, aiming for residents to remain through the end of their lives, although we cannot promise this as care needs might exceed our capabilities. Third-party services like PT, OT, in-home health care, and Hospice help support this process. Our waitlist is typically active for all apartments in Assisted Living and Memory Care, and it varies based on the apartment size and the area of interest.
