Overview of Staff and Training

Trilogy is committed to annual, ongoing training as required by the state to ensure that our residents' needs are met, safety measures are in place, protocols are understood, etc.

Safety and Well-being Measures

The care of each of our residents is based upon their individual care plan meetings, as well as doctors’ orders. This provides each of our campus disciplinary teams (nursing, dietary, etc) the opportunity to ensure that all needs are met and their safety is of the utmost importance.

Transition Support Services

Our entire team including residents help to make our new residents feel welcomed. Our Life Enrichment Team provides purposeful and engaging activities. We encourage our residents to get involved. We follow up with new residents and families to ensure they are transitioning well.

Transportation Services

We can provide transportation whether that be via our activity bus or a 3rd party transportation service. Fees may apply.

Dining Experience

Our expert chefs use fresh, local ingredients to create delicious and nourishing meals that cater to a wide range of dietary needs and preferences. Features include restaurant-style dining, café menu featuring resident-focused regional favorites, hand-dipped ice cream, juice, coffee, and Coca-Cola products available any time. Other favorites include Holiday Events, monthly Sunday Brunch, Taste of Town, Quarterly Themed Dinners, use of private dining room for special events, and more.

Addressing Complaints and Issues

We welcome our residents or families to express any concerns or complaints they may have. Using an electronic entry form, any staff member can enter a resident's concern. A team member, depending on the department, will follow up with the resident or family to address. We also hold Resident Council meetings at our facilities to provide input on various items like meals, activities, etc.

Cost of Living

We are a completely skilled community. Our rates start at $339/day. Our memory care semiprivate accommodations are $289/day. Level of Care (LOC) is billed in addition and is based upon how much assistance the resident needs from our clinical team. It ranges from $29-89/day.

Communication with Residents' Families

Care plan meetings and continuous follow-up calls throughout your loved ones' stay. If there are any concerns, emergency contacts are notified.

Short-term or Respite Stays

Depending on availability, we can offer short-term/respite stays. Rates are the same per day with LOC charges.

Pet-Friendly Community

Pets are welcomed to visit inside the residents' room. We do require up to date vaccine records on file.

Waitlist Information

We currently have a waitlist, but this can change day to day. The waitlist depends on different factors. For example, if a resident is needing Medicaid options, there is a waitlist which can average a year+ at many of our campuses.

Vaccination Services

Yes, we offer Covid, flu, and pneumonia vaccinations.

Aging in Place

We are a completely skilled campus, offering the highest level of care. Serving our residents needing short-term rehab, long-term care, and skilled memory care. However, we do have sister campuses that offer continuum of care options, meaning our residents will never have to move again if their health needs change.

Our Services

We are a skilled care facility that can accommodate residents needing a short stay for rehab or respite care. In addition, we can accommodate long term stays for residents needing assistance with activities of daily living, such as bathing, dressing, and more.

Legacy Wing

We also have a Legacy Wing that provides a secure environment with purposeful activities for our residents who suffer from dementia as well as support to their families.
