Community Details

Community Details

This detailed information was received through an email from the Marketing Director at Pacifica Senior Living San Martin via email on 2023-11-17.

Overview of Community Services

I will answer as best as I can, a lot of these questions will vary from community to community. You have to understand that we are not a hospital or skilled community. We do not follow the same guidelines as a hospital would. WE ASSIST OUR RESIDENTS. This building is an Independent, Assisted Living, and Memory Care community.

Medical and Care Services

24-hour care staff, med techs, wellness nurses, also we have three in-house Doctors that are independent, they take all insurances. A lot of our residents will use them for a primary so we have access to them 24 hours a day. They ARE NOT STAFFED BY PACIFICA.

Safety and Emergency Preparedness

The safety and well-being of all of our residents is our utmost importance. We have emergency drills monthly, we are prepared for any major disasters, if should occur. All residents receive pendants to wear in case they fall or need assistance, even if you are independent. The pendant tells the care staff where you are anywhere in the building.

Orientation and Transportation

All staff will make sure for the first couple of weeks that each new resident is escorted to the dining room and to all activities. It also depends on the new resident what they need and what kind of assistance they are on. Each person is different.

We have a bus and a bus driver that’s available Monday – Friday ONLY. We have medical transportation days Tuesday, Wednesday, and Thursday, all appointments must be arranged with our wellness nurse Rosi, as our bus driver can only drop off and pick up. All outside Dr appointments must be within 15 miles of our community. We have shuttle service to the grocery store every Monday after breakfast. Bus will take them to Walmart etc. again he will drop you off then come back at a designated time and pick everyone up. Every Friday are casino days, whoever wants to go spend a few hours at the casino, we will drop off and pick up.

Dining Experience

Our dining room is restaurant style. Open seating they will come take your order. Breakfast you can have whatever you want. Lunch and dinner there is always two Entrees to choose from. If it's something you do not like, just ask the waiter, you can always have an alternate meal. If they have it they will cook it for you. Breakfast is from 7-9am, lunch is 11-1pm, dinner is 4-6pm.

Communication and Management

Depending on the question, we have certain directors for certain departments. If it's dining, the dining director, issues with rent etc – Business office manager, anything to do with care, our wellness nurse. Also, the Executive Director's door is always open for any questions you may have.

Cost of Living and Care Levels

As far as cost of living, all prices were in our rate sheet I sent you. Only thing different between our independent and assisted living prices is if you are on a care level or not. If you come in not needing any kind of help and can do everything on your own, then you just pay the rent prices listed on top. If you come in needing help with any of your daily living needs, bathing, meds, grooming etc. then you pay your rent PLUS A CARE LEVEL LISTED AT THE BOTTOM. Everyone will be assessed by Rosi. The care levels are done by a point system. That amount will be determined when Rosie sits down with you and discusses what you may or may not need. Also, what is nice about our community, you can come in as independent, but down the line if you start needing assistance it is available to you, and you do not ever have to leave the apartment you're in. Some communities that are independent and assisted living will be separate, so if you start off independent and need assistance later you have to move. Here you do not do that.

Visiting and Family Communication

Families are welcome to call and come and go here. There are no visiting hours. Our care department will always keep the families updated on change of conditions. if the resident is showing signs of decline, they fall and are sent out to hospital etc, the wellness department is notified. When you move in there is emergency paperwork that families need to fill out for this one reason. We also do a 911 packet for all emergency responders in case of emergencies.

Leasing and Respite Care

We are a month to month community, you sign no 6 month or year lease, all we ask is that if you decide you want to move out you give us a thirty-day notice. If a resident passes away, you may move out within 14 days. Some families like to move out immediately, some like to wait until the end of month. It is up to the family. And we will prorate the month accordingly.

We do have respite, but it all depends on what kind of care you need. It is a minimum of a 15-day stay. It can start for assisted living side from 150 a day to 300 a day, again, depending on what care level you are at. Everyone even coming in as a respite, will have to fill out and complete all paperwork like they are moving in. Pacifica paperwork, all physician reports need to be filled out by a Dr. assessments have to be done. etc.

Pet Policy

We are pet friendly. All residents MUST BE ABLE TO TAKE CARE OF THEIR OWN ANIMAL. Our care staff cannot help with this. We have a doggy run for them. If the resident begins to show signs of neglect with the animal, aka, not taking for a walk, cleaning up after them, making a mess in the apartment, we will then have a conversation with you and your family about taking the animal away. There is also a one-time $500 pet fee. All dogs cannot be over 35 pounds.

Availability and Incentives

We do not have a waitlist. Have one bedrooms and a few studios available. Also have some great incentives going on until the end of the year.
