RATE SHEET: ODD FELLOW REBEKAH HOME

BASE RATE (per person):

Harmony Center (Alzheimer’s Care)

The above charges include room and board and basic nursing care. The bed-hold rate is 75% of the daily base room rate. Arrangements can be made for temporary/respite care, not to exceed 14 consecutive days. This service offers nursing care and support services when primary caregivers are unable, absent, or in need of temporary reprieve of caregiving duties. The rate is fifteen dollars ($15.00) per day added to the base rate quoted above.

ADDITIONAL SERVICES

Nursing Supplies and Equipment: Nursing supplies are built into the cost of the base room rate except for oxygen, oxygen concentrators and resident specific specialized equipment which will be billed monthly.

Other Charges: Beauty shop, barber charges, transportation services, guest meals, and personal purchases are billed monthly (contact facility for services provided), as used, unless facility requires pre-payment of services. Contact facility for details. Physical therapy and/or other therapy ordered by the Doctor will be billed monthly. Doctor fees and prescription drugs are billed directly by the supplier. Rates are subject to change upon notification. Room and board is payable in advance and is only refundable on medical discharge.

Day Care: Available from 7:30am – 5:00pm (negotiable) (Includes recreational therapy, personal care, and supervision) $72.00 per day (maximum of 8 hours); $15.00 per hour for every hour after 8; 1 meal and 1 snack provided at no cost, $5.00 extra for additional meals. NOTE: If the individual is still in the facility at midnight; they are considered to be an in-house resident and will be charged room and board for the day instead of the Day Care rate.

Contract Rights: All the rights and responsibilities of a resident pass to the resident’s guardian, next of kin or sponsoring agency or agencies if the resident is adjudicated incompetent under State law or is determined by his/her physician to be incapable of understanding his/her rights and responsibilities.

Room Availability

We are currently operating off of a wait list for our rooms, which includes rehab to home, long-term care, and memory care. Availability can change from day to day, and we always encourage you to schedule a tour.
