First level of care: $235.00 per day

• Resident may require wheelchair
• Resident requires minimal assist of one staff with ADL’s
• Resident requires medication management of less than 15 medications
• Resident requires some assist with showers twice a week

Second level of care: $265.00 per day

• Resident has a dementia diagnosis
• Resident requires incontinence care (incontinence products included in daily rate)
• Resident requires assist with feeding
• Resident requires medication management of more than 15 medications

Third level of care: $290.00 per day

• Use of personal body alarm or fall alert device including fall mat
• Any medical equipment (etc. oxygen, special air mattress or other doctor prescribed equipment)
• Resident has a lift
• Resident is a 2-person transfer
• Resident has a feeding tube
• Resident needs wound care or ostomy care
• Resident needs blood glucose checks
• Resident receives Hospice Services

All the above levels are all-inclusive of the following items:

• Room and Board
• Three meals plus snacks
• All personal care items such as toothpaste, shampoos, soaps, deodorant, toilet paper, toothbrushes, etc.
• Assistance with all activities of daily living as needed.
• Laundry service
• Hospital Beds and bedding, towels
• Cable Television

Items excluded would include all things physician prescribed such as prescriptions, medication, medical equipment, and personal items such as eye glasses, hearing aides, and hearing aide batteries, dentures, and clothing.

*Discount for couples - $200.00 per month*

Transfers are $50.00 round trip and include drop off and pick up. Family member or other responsible party are required to meet resident at appointment.
