When using a pharmacy other than the one integrated with Oak Terrace, a $200/month fee will apply. Medications that require daily weights or weekly blood draws may incur an extra charge of $100 per month.

The care level will be set by Oak Terrace's nursing team. As a strong guideline, the highest care level in which a resident receives services will be used.

Level 1: $4350

• Minimal assistance and/or supervision with ADL's
• 24 hours staffing
• Daily activity schedule
• Monthly blood pressure and weight monitoring
• Weekly laundry services
• Housekeeping services
• One supervised shower/whirlpool bath each week
• Needs no assistance with personal transport
• Continent
• Requires redirection but is easily redirected
• Little to no behavioral problems and/or impulse control issues
• Can communicate needs with staff

Level 2: $4900

• Frequent hands-on assistance with ADL's with resident assistance
• Occasional assistance with personal transport
• Occasional incontinency, minimal assistance with bowel/bladder program (2 or fewer x week)
• Occasional behavioral problems/difficulty with redirection
• Special dietary needs (pureed, thickened liquids)
• Cueing or reminders needed to feed self
• Non-insulin dependent diabetes

Level 3: $6000

• Moderate to extensive assistance with ADLs on a daily basis, minimal resident participation
• Regular assistance with personal transport
• Hands-on assistance of one staff member for transfers or walking
• Incontinent, assistance with bowel and bladder program
• Needs redirection and/or reassurance multiple times per day
• Occasional physically aggressive and/or sexually inappropriate behaviors
• Assistance with feeding
• Insulin-dependent diabetes

Level 4: $7000

• Total assistance with a majority of ADLs
• Requires staff to reapproach multiple times to perform cares
• Hands-on assistance of two staff members for transfers, walking, or use of a mechanical lift
• Daily licensed nursing staff interventions - catheter, wound care, O2
• Needs redirection and/or reassurance consistently throughout the day
• Full assistance with bowels and/or bladder program
• Requires feeding assistance during meals
• Requires frequent one-to-one supervision

Level 5: $7500

• Requires total assistance with all ADLs
• End-of-life care and coordination with hospice
• No longer eating (will not require meal plan)

• Standby assistance with transfers
• Occasionally incontinent requiring staff assistance (less than a daily basis)
• Overnight checks by staff
• Daily reminders of meals, appointments, and activities
• Non-insulin dependent diabetes with assistance needed for accuchecks

Level 3: $4000

• One person assists with transfers or walking
• Escort to meals on a frequent basis
• Moderate hands-on assistance with Activities of Daily Living on an ongoing basis
• Assistance with toileting daily or frequent incontinence
• Special dietary needs
• Frequent Call Light Activity or repetitive anxious requests
• Occasional redirection for socially unacceptable/inappropriate behavior
• Frequent need to contact/coordinate with physicians for order changes and/or changes in condition (greater than 1x/week)
• Insulin, nebulizer, oxygen

Level 4: $4900

• Two-person transfers or Mechanical Lift assistance for transfers
• Extensive assistance with Activities of Daily Living (with resident able to assist)
• Colostomy or Catheter care that requires staff assistance
• Daily licensed nursing staff interventions - catheter, ongoing wound care, respiratory management

Level 5: $5800

• Resident is unable or limited in their ability to assist with activities of daily living
• Resident is not able to communicate their needs to staff
• Resident requires staff to reapproach multiple times to perform cares
• Resident requires frequent repositioning by two staff members
• Resident is incontinent of bowel and bladder, requiring two staff to attend
• Resident requires feeding assistance

Additional Person (assisted living) $1200 plus applicable level, med fee, and meal plan

We have openings in our Independent Living units.

Assisted Living

We have openings in our Assisted Living units, but are currently full for any EW/CADI accommodations.

Memory Care Unit

We have openings in our Memory Care unit, which would be a companion situation.

EW/CADI Accommodations

We offer EW/CADI accommodations in either a studio or shared room. Currently, we are full for EW/CADI in Assisted Living but have availability in our Memory Care unit.
