In Nakoma, we have a range of skilled professionals dedicated to providing quality care. Our team includes:- Skilled Nursing/Emphasis on Rehab: Led by a Director of Therapy, our therapists specialize in Physical, Occupational, and Speech Therapies.
- Nursing Department: This includes a Director of Nursing, Admission RN, and a Social Worker.
- Certified Nursing Assistants (C.N.A.s): They assist with the necessary care for each of our patients.

Measures to Ensure Resident Safety and Well-being
Each resident undergoes an assessment and we collect medical information from their primary physician prior to admission. We understand the specific diagnoses and support each resident requires and communicate this to our care staff. Each resident wears a call pendant for additional support throughout the day and night. Our Activity Director and staff also learn about residents' personalities and care needs to ensure comprehensive support.Resources and Support Services for Transition into the Community
Our Director of Sales learns about potential residents during the initial meet, and this information is shared during daily Stand Up/Stand Down meetings. Our staff welcomes new residents, and we continue to learn and share information about them during Shift Change. The Activity Director and aides also help with introductions to other residents and encourage participation in activities.Transportation Services
We offer transportation for medical appointments.Dining Experience
We have a 5-week menu rotation, which is completed weekly. Our Activity Aides assist residents with the menus. We offer a main entrée and an "always available" menu. Residents are encouraged to dine in the dining room for social interaction. Directors help in the dining room by monitoring, socializing, and sometimes serving. Tables are set for four residents each, and background music is sometimes played. We accommodate special diets as well.Handling Resident Complaints or Issues
We have a Concern Form that can be completed by any resident, family member, or employee. Our Administrator and Nurses work together to address concerns and find resolutions. The form is also used as a training tool.Cost of Living
Costs reflect the level of care:- Assisted Living: $7451 to $9315 monthly for a studio or 1-bedroom, and there is also a 2-bedroom option.
- Memory Care: $8538 to $9837. Rates are all-inclusive.
