Assisted Living, Memory Support, and Skilled Nursing Facilities

As noted in my initial email, we offer assisted living (AL), memory support (MS) and long-term skilled nursing (SNF). We do have licensed nurses and certified nursing assistants 24/7.

Person-Centered Care

We are a person-centered facility, so we have a team who works with the family.

Application Process

Anyone who is interested in pursuing long-term placement has to submit the application and the required paperwork. We then submit these to a review committee. Anyone who is accepted long-term is able to transition to both Medicaid and LT nursing when they get to that point.

Transportation Services

Transportation can be arranged for a fee.

Dining Services

We have a restaurant for our AL residents, and each nursing floor has their own dining room. The residents’ dietary restrictions, if they have any, are considered for their meal plans.

Communication and Care Planning

We always encourage our residents and their family to talk to our social worker or nursing staff if they have concerns. All our residents have periodic care plan meetings.

Rate Information

I sent you the rates for our AL services, please find attached the rates for MS & SNF. The rate sheets also include the respite rates. Respite stays require a minimum of 5 days, but no longer than 60 days. There is also a one-week security deposit required for this.

Pet Policy

As long as residents can take care of their pets, they are able to keep them in AL, but not in MS & SNF.

Admission and Wait List

We do not have a wait list. Acceptance hinges on the clinical assessment and committee review of assets.
