Monthly Room Rates

Level of care will be assessed prior to move-in. Rates below.

Includes:

• Medication management, healthcare management, and supervision
• 24 hours per day care and security
• Three chef-prepared meals per day
• Daily social activities and outings
• Weekly housekeeping and laundry
• All utilities except phone
• Private rooms with private bathrooms, kitchenettes, and individual climate control.
• Care services provided at an additional cost based on the assessment.

Community Fee on all resident admissions – A one time fee of $1500

Respite Care – (30 day minimum) Assisted Living - $220 per day

Levels of Care

Prior to admission, an assessment will be performed to determine the level of care at admission. This rate will be added to the base room rate. Residents will be re-evaluated if there is a significant change in condition after admission.

Level One: $500 per month

Includes all monthly services and the following:

• Assistance with grooming and dressing at a supervision or minimal assistance level
• Shower assistance for safety and thoroughness
• Escort to dining room and recreation room
• Diabetic Management (Blood Sugar Monitoring and Insulin Management)
• Assistance with Oxygen Management
• Assistance with Foley or Ostomy Management
• Assistance with BiPAP or CPAP
• Wound care
• Hospice Services

Level Two: $1000 per month

Includes all monthly services and the following:

• Includes combination of medical and ADL services listed above in Level 1.

Level Three: $1500 per month

Includes all monthly services and the following:

• Assistance with grooming and dressing at a moderate to maximum assistance level
• Wheelchair transport assistance or escort
• Transfer assistance.

Level Four: $2000 per month

Includes all monthly services and the following:

• Includes combination of medical needs (listed in Level One) and moderate to maximum assistance with ADL’s
• Wheelchair transport assistance or escort
• Transfer assistance

Level Five: $3000 per month

Includes all monthly services and the following:

• Assistance with ADL’s
• Two persons assist for transfers
• Management of medical needs
• Wheelchair transport assistance or escort

Memory Care

• Large Studio Starting at… $7400.00
• One-Bedroom Studio Starting at… $8200.00
• All inclusive (No Level of care fee applied in Memory Care)
• Related second person fee additional $1500.00

• 7th Floor Apartment $3000 to $3500 (Includes a full kitchen)

We are conveniently located on McKnight Road in the North Hills and offer transportation to doctor appointments, scheduled shopping trips, scheduled activities to enhance socialization and life enrichment, meals, housekeeping, laundry, and more.
