Do you guys currently have a waitlist?

We do have a waiting list based on immediate need and pay source.

Could you share pricing and any other general information?

Our current pricing is $278.00/day.

Could you provide an overview of the types of professionals on your staff and the training they undergo at your facility?

We have RNs, LPNs, Certified Nurse’s Assistants (CNAs), Physical Therapist, PT Assistants, Occupational Therapist, OT Assistants, Speech Therapist, degreed Social Workers and all employees are assigned continuous education, trainings annually and quarterly, CPR certification/recertification, and any education needed for license renewals etc.

Could you tell me about the measures you have in place to ensure the safety and well-being of your residents?

That is more than responding to an email could possible entail, but we have wander guards for exit seeking patients which cause our doors to lock when approached, we have tons of processes and policies all employees adhere to in order to ensure patient safety. Our purpose is quality of life and we have a life enrichment director/dept to ensure engagement and ongoing positive well-being.

Do you offer any resources or support services to aid residents' transition into the community?

When we have SNF patients discharging to the community we have a discharge planner in social services that facilitates all equipment and services needed.

Do you offer transportation services for residents?

Transportation to and from appointments can be arranged.

Could you tell me a little about your dining experience?

We have a full service dining room and assistance is based on the patient’s need. We offer a menu that allows the patient/resident to choose their meals.

What is the process that takes place if a resident has a complaint or an issue they want addressed?

We have a process we use called “service recovery” as part of our grievance process. I (Laura Tapp – Director of Social Services) serve as abuse coordinator and 1st line in the grievance process. We use an independent company for a survey process which allows patients and families to express their opinions about the care the patient is receiving.

What is your policy on communication with resident's families?

I can’t say that we have a “policy” but it is our intention to have continuous communication. We have an “open door” policy at all times. We have facilitated meetings within 72 hours of admission called a “Focus Meeting”, we have a “Care Plan” meeting around the 21st day, then depending on the discharge plan or transition plan for the patient we may have a discharge planning meeting or quarterly care plan meetings for long term patients.

How do you keep them updated about their loved ones' well-being?

Families are contacted for any change in condition, med changes, the above mentioned meetings, weekly if patient is here short term and we are a “small town” facility so anytime a family member visits we can be available for communication.

Do you all offer short-term or respite stays?

We do offer short term/respite stays. These have to be very planned in advance as we stay at capacity a lot.

How much does this cost?

The cost is the same for short stay/respite.

Are you all a pet-friendly community?

As long as the shot record can be produced and we determine the pet is not a possible harm we are VERY pet friendly.

Are residents able to get Covid vaccination and flu vaccinations in the community?

We are able to give COVID and Flu vaccines.

Are residents able to age in place?

We are a skilled nursing facility as well as a long term care provider so yes, they can age in place. We do have a waiting list based on immediate need and pay source.
