Community Details
This detailed information was received through an email from the Sales Coordinator at Nazareth Living Center via email on 2023-11-14.
Overview of Professional Staff and Training
For medical, we have RN's, LPN's, certified nurse's aides and caregivers. They are all highly trained from schools and have ongoing education courses several times a year.
Other staff and departments include administration, activities/wellness coordinators, spiritual care coordinators, and culinary among others.
Safety and Well-being Measures
The residents in assisted living wear emergency pendants around their neck if they need immediate assistance. All residents will have a daily well-being check. There are staff in the building 24/7.
Transition Support Services
We do not have moving services, unfortunately. All new residents are welcomed into the community by staff and other residents. We have a strong support team to help the transition go as smoothly as possible. We offer daily mass in our beautiful chapel 5 days/week and on Sundays.
Transportation Services
Yes, medical transportation is provided for doctor's appointments. Our transportation will drop you off and pick you up. Unfortunately, they are unable to stay with you at your appointment. Any appointment that is over 12 miles one way will be charged an additional $25. This will reflect on the monthly bill.
Dining Experience
In assisted living, our residents are served meals three times per day; 7:30, 11:30 and 4:30. The largest meal is during lunch. We have daily specials and an alternative menu. These meals are served restaurant-style.
Resident Complaints and Issues
Assisted living has a housing manager that will address any complaints/concerns that you may have, privately. There is also a monthly resident meeting for assisted living that is run by the Executive Director and managers of most all departments. These meetings are to keep residents informed of happenings in assisted living and also to address any concerns that may arise.
Cost of Living
I'm not exactly sure what you mean by cost of living. Prices tend to increase once yearly, the percentage of increase may vary in each of our communities (independent living, assisted living, memory care and long-term care).
Communication with Resident's Families
All residents have a face sheet, similar to a hospital, that has emergency contact numbers for family/relatives. The nurses will contact the emergency contact on file should you have any incidences. Each resident will have a service plan that will be conducted by the Director of Nursing. Families may be involved in this meeting. Your needs will be addressed and/or updated in the meeting. Family members are more than welcome to call anytime for updates on their loved ones.
Short-term or Respite Stays
We do provide respite stays. I have attached the price sheet. For short respite stays, we can provide a private room without a shower. Rate is daily and it is $412/day
Pet-Friendly Community
Yes, for assisted living and independent living residents only. Currently we have an approximate two-year waitlist for both options.
