Community Details

Community Details

This detailed information was received through an email from the Director of Community Relations at Monona via email on 2024-08-06.

Overview of Staff and Training

Our staff includes the Director of Nursing (RN), Assistant Director of Nursing (LPN), 2 clinical supervisors (LPNs), Executive Director, 2 Assistant Directors, Life Enrichment Director, 2 Life Enrichment Coordinators, Certified Nursing Assistants (CNAs), and Residential Care Assistants (RCAs). We follow state guidelines for training new employees and provide 15 hours of continuing education annually.

Safety and Well-Being Measures

Our building is locked in the evening, with residents using key fobs for entry. Visitors are monitored via a ring doorbell with camera and audio. We conduct routine resident checks based on their Individual Care Plan, ranging from every couple of hours to every half hour. Our clinical team operates on a rotation schedule during weekends, and RNs or LPNs are on call 24/7.

Support for Residents' Transition

We coordinate with the Aging and Disability Resource Center (ADRC) for resources. New residents receive a welcome sheet introduced to the team and encouraged ongoing introductions until they become familiar with the staff.

Transportation Services

We offer a mini-bus for group outings but not for personal appointments. We assist families with transportation arrangements if needed.

Dining Experience

Residents enjoy breakfast, lunch, dinner, snacks, and hydration. We offer a main entrée and an alternative, with everyday items also available. Meals are mostly homemade from scratch and rotate on a 5-week schedule.

Handling Complaints and Issues

Residents can speak with our director regarding complaints. We have a state-regulated grievance policy outlining steps to take if the resident isn’t satisfied with their discussion with the director.

Cost of Living

Costs depend on apartment size and individual care plans, averaging between $5800 - $7800.

Communication with Families

A contact list is established upon moving in, detailing who can be contacted and who can access protected information under HIPPA laws.

Short-Term or Respite Stays

We offer short-term or respite stays, costing $200 or $250 per day plus care costs.

Pet-Friendly Policy

Yes, we are a pet-friendly community.

Waitlist Status

No, we currently have available apartments.

Vaccinations

We provide flu vaccinations annually. Uncertain about COVID-19 vaccinations.

Aging in Place

Our goal is to provide care so that residents do not need to move again. With in-home care agencies, hospice agencies, and our on-site physical department, most residents stay through end of life without transferring to a skilled nursing facility.
