Meal Plan Options

• Three (3) meals each day for $180.00 per month.
• Two (2) meals each day for $150.00 per month.
• One (1) meal each day for $90.00 per month.
• No meal plan through Heritage Hall.

Ala carte meals are $5.00 per meal if you eat a meal outside of your meal plan

STANDARD AMENITIES AND SERVICES INCLUDED IN ALL PACKAGES

• Kitchenettes

REQUIRED MONTHLY BASIC SERVICE FEE $250.00 PER RESIDENT

Includes the following services:

• Twenty-four (24) hour on-site home care staff
• Remote activated call system monitored by on-site staff 24-hours a day
• Health and Wellness Clinic
• Daily “I’m O.K.” check of residents

PACKAGES FOR ASSISTANCE WITH ACTIVITIES OF DAILY LIVING

Package A 0-30 minutes per day $700/month

Package B 31-60 minutes per day $1275/month

Package C 61-105 minutes per day $1850/month

Package D 106-120 minutes per day $2425/month

REQUIRED HOME CARE NURSING SERVICES

Initial Admission Assessment $300.00 each

Readmission assessment following inpatient stay $200.00 each

Re-assessment following a fall $55.00 each

OPTIONAL HOME CARE NURSING SERVICES

Nurse consult or assessment $50.00/15 minutes

Lab Draw by MMH Nurse $40.00/draw

Catheter Care $350.00/month

Blood Sugar monitoring: Level 1 (1-2 times a day) $400.00/month

Level 2 (3-4 times a day) $650.00/month

MEDICATION MANAGEMENT INCLUDES SETUP AND ADMINISTRATION

Level One – Up to Eight (8) Medications $800.00

Level Two – Greater than Eight (8) Medications $1000.00

*Medications include prescription and all over the counter medication and supplements.

OPTIONAL HOME CARE SERVICES / HOME HEALTH AIDE

Shower, sponge/bed bath $30.00 per visit

Additional Home Health Aide visit (unscheduled) $20.00 per 15 minutes

Whirlpool bath (45 minute minimum) $30.00 per bath

Escort $15.00 per trip

Use of Protect Alert Response $25.00 per 15 minutes

Laundry (separate loads) $25.00 per load

Two loads a week/eight loads a month $125.00 per month

Ironing $15.00 per 15 minutes
