Staff
We have a wide variety of staff here to meet different patients' needs. It really varies whether you are looking for rehab vs long-term care. Regarding our administrative staff, we have licensed social workers, a dietician, Medicaid technicians that work in our business office that help with the insurance aspect for coverage, admissions staff like myself, and obviously our administrator as well as two DON's. On the floor, we have RN's on each unit as well as LPN's and nurses' aids. We also have our own housekeeping staff and kitchen staff. As far as nursing goes, each of our nurses shows proof of licensing before hire. We additionally have a wound care team to assess our rehab patients and prevent pressure ulcers for our long-termers.Quality Measures
We have personalized care plans for each patient and different programs like caring partners and resident voice to catch any possible concerns/issues.Community Resources
We have a discharge planner that works with different community resources like the Info Center to find appropriate housing/transport/and/or equipment needs when going elsewhere.Transport
We have our own wheelchair vans to transport residents to appointments. If a pt requires a stretcher for transportation, we have a few companies that we work with that can assist.Dining
We have a dietician onsite so she can assist with any dietary needs. Our meal times are 8am, 11am, and 5 pm. They can get meals outside of those times, but it might be something smaller. We have great kitchen staff and the menu is posted outside the dining room.Complaints
Every resident is assigned to a department head. Each week we meet with them to get any issues/concerns. We also have quarterly care conferences with them and the whole team so every department can give input.Cost of Living
I am not sure what exactly you are asking, but our prices are as follows: Private rooms: 300 per day. Semi-private with one other person: 275 per day. This is only in question when there is no insurance covering their stay at all.Family Communication
Families are allowed to visit with the hours of 8am-8pm. If a pt is not their own person (is not cognitively intact) we communicate any plans of care/issues with their decision-maker whoever that may be. Regardless of the decision-maker, families are always allowed to voice their concerns or ask us questions and we will answer to the best of our ability. We do offer respite stays. Sometimes insurance will cover, but if not, it is the same rates as listed above. Either the 300 or 275.Pets
Pets are welcome to VISIT as long as we get proof of vaccination.Bed Availability
We do not have a waitlist at this time, but bed availability fluctuates daily.Vaccinations
We provide any vaccinations in-house. We have an infectious disease nurse and she can provide any flu, COVID, influenza, or other vaccines with the patients' consent.Aging
I am assuming you are referring to Long Term Care. Yes, we do offer rehabilitation as well as long-term care. Medicaid does cover long-term care services.
