Director of Nursing

We have a Director of Nursing who is an RN and CNA. The CNA Staff is here 24 hours.

Wellness and Safety Features

We do wellness checks and medication reminders. We also have emergency pull cords in the bathrooms and an emergency alert panel on the wall by the residents' bed. If they pull the emergency pull cord or push the alert panel near the bed, staff will respond ASAP.

Furnishing for Residents

If residents do not have furniture, we will provide a twin bed, TV, small dresser with cloth drawers, and a small dining set. The furniture is a nice set of starter furniture which residents can replace anytime, and we will take ours out.

Transportation Services

Proviso Township provides rides for residents. Our Social Worker works with residents to coordinate rides. We have a bus as well, transporting residents to local stores like Walmart, Family Dollar, Dollar Tree, or wherever they ask us to take them within reason.

Apartment Amenities

Supportive Living provides a small refrigerator and small microwave in the apartments, as there are no stoves. We provide meals for residents: breakfast, lunch, and dinner restaurant-style dining. We serve residents with open seating in the dining room. Residents can have an air fryer, coffee pot, crock pot, and roaster, just NO HOT PLATES.

Grievance Procedure

Residents can put in a Grievance. We will sit with the resident to try and resolve issues or come up with a viable solution.

Income and Services

Maywood Supportive Living is a low-income community. The income limit for 1 person can’t exceed $43,800.00 annually. Residents keep $90.00 of their income a month, and we provide all services, including meals.

Family Communication

You are welcome to call in and check on your loved ones. However, if we have concerns and if they are sent out of the community to the hospital, we advise the family.

Lease Terms

No short-term or respite stays; leases are 12 months. We do, however, have a short waiting list right now. If interested, I would get the process started ASAP.
