Private Pay Daily Rates

Our private pay daily rate is $330/day for long term and $360/day for short term skilled.

Overview of Professional Staff and Training

Our facility is staffed with a diverse range of professionals including physicians, nurses (RN and LPN), STNAs, physical therapists, and occupational therapists. All staff members have completed the necessary schooling and classes for their respective roles. Additionally, we provide various training sessions as refreshers to ensure our team is up-to-date with the latest practices and procedures.

Safety and Well-being Measures

To ensure the safety and well-being of our residents, we have implemented several measures. This includes a secured unit for dementia residents at risk of elopement, the use of wander guards for some residents, and alarms on doors to monitor and control movement.

Transition Support Services

We assist residents in transitioning into the community by setting them up with home health care and home health nursing upon discharge, ensuring they continue to receive the care and support they need.

Transportation Services

While we do not have our own transportation services, we can arrange transportation with a company. This may incur a cost to the resident if their insurance does not cover it.

Dining Experience

Our dining experience includes restaurant-style dining in our dining rooms, with the option for residents to eat in their rooms if they prefer. We offer a set menu for each meal, along with a favorites menu for residents seeking alternative options.

Complaint Resolution Process

Residents can express their concerns through a complaint form, which is then submitted to the administrator for review and action. This ensures all issues are addressed promptly and effectively.

Cost of Living

The cost of living in our community is $330 per day for long-term stays and $360 per day for short-term skilled stays.

Communication with Residents' Families

We maintain open communication with residents' families, notifying them of any changes in the resident's condition. Families are also welcome to call for updates and are invited to attend care conferences held every quarter for long-term residents and upon admission for new residents.

Short-term and Respite Stays

We offer short-term skilled stays and respite stays at a rate of $360 per day for short-term skilled and $330 per day for long-term or respite stays.

Pet-Friendly Community

Pets are welcome to visit provided they are up to date on vaccinations, ensuring a friendly and inclusive environment for all residents.

Vaccination Services

Residents have access to Covid and flu vaccinations within the community, supporting their health and well-being.

Aging in Place

Our community supports residents in aging in place, offering the necessary care and services to accommodate their changing needs over time.

Waiting List Status

Currently, there are 4 individuals on our long-term care waiting list, indicating the demand for our services.

Facility Overview

We are a 100-bed facility with all private rooms and private bathrooms. Out of those 100 beds, 17 are in a secure memory care unit.

Therapy Gym Features

We have a 3,000 square foot therapy gym that has a full kitchen, laundry room, and bedroom so our short-term rehab patients can practice their ADLs (Activities of Daily Living) prior to discharge.
