Included in the monthly rental rate:

• Select from a menu of choices in the Mala Strana Assisted Living Dining Room. Fine dining, served directly to your table. Special diet requests are always honored.
• ★ A weekly 1⁄2 hour of housekeeping provides you with more leisure time.
• ★ A personal call system that you can carry with you. Help is just the push of a button away.
• ★ No need for coins, laundry facilities are complimentary.
• ★ Enjoy participating in a wide variety of social events and activities.
• ★ Keep your apartment as warm or as cool as you would like it. All utilities (except phone) are included. Heat and central air conditioning are controlled via thermostat located in your apartment.
• ★ If the need should arise, you have access to Physical Therapy services at the Mala Strana Rehabilitation Center.

Laundry: 2 loads of laundry per month are included, extra loads are $12 / load. This service includes; gathering laundry and supplies, washing, drying, folding, returning and putting laundry away. The bed will be made when bed sheets are laundered.

Guests are welcome to join for meals, which are $10.00.

The Next Step

Once you have made the decision to become a resident of Mala Strana Assisted Living:

• ★ A $1,500.00 reservation fee reserves the apartment of your choice. At the time of move-in, this fee is applied as a security deposit.
• ★ Prospective resident must contact Senior Linkage Line for mandatory transitional counseling and obtain a validation number prior to admission to an assisted living setting in Minnesota.
• ★ Move-in dates are arranged.
• ★ Prior to your move-in, the Nurse Manager will set up a time to meet with you and your family. This is a good time to collect general information and past medical history. A services plan is discussed and agreed upon to ensure your comfort and safety. Social Services will advise you in planning your move and introduce you to your new home and friends when you move in.

Assisted Living Packages

Mala Strana Assisted Living shall provide assistance as needed, twenty-four hours per day, seven days per week, to residents who subscribe directly for such services. Listed below are the package levels, which are determined by the hours of services provided in a month, based on the resident's assessed needs. The resident's package level and services are detailed on a Service Plan Agreement that is entered into by the resident or a designated representative. Service plans and package levels are periodically evaluated according to changes in the resident's needs and level of service provided. If less than 5 hours of service a month is received, then there is a charge for each service.

• Level 1: 5 - 20 hours of service per month $1,120.00
• Level 2: 21 - 30 hours of service per month $1,625.00
• Level 3: 31 - 45 hours of service per month $2,070.00
• Level 4: 46 - 60 hours of service per month $2,550.00
• Level 5: 61-75 hours of service per month $3,050.00
• Level 6: 76 - 90 hours of service per month $3,495.00
• Level 7: 91 - 105 hours of service per month $3,910.00
• Level 8: 106 - 120 hours of service per month $4,345.00
• Level 9: 121-135 hours of service per month $4,755.00
• Level 10: 136-150 hours of service per month $5,190.00
