We accept Medicare, Medicaid and Private Pay.

A Semi-Private Room Rate is $225/day.

Our Caregiving Staff

All caregiving staff are licensed (CNA’s, LPN’s, RN’s). Most licensed staff do CME (Continuing Medical Education) classes. Our Administrative Staff are trained in their specific Department Specialties (Admissions, Social Services, Business Office, etc.). We complete training and education through Relias, an online program.

Resident Safety

Each resident's safety needs are different and as safety concerns become an issue the appropriate safety precaution would be put in place. For example, for a fall risk, bed at lowest position and 15-minute checks would be appropriate.

Support and Medical Oversight

We have an Administrator over the residents and staff that offers support and encouragement. Our Attending Physician as well as our Nurse Practitioner oversee the resident from a medical standpoint. We also have professional services offered through our Psychiatry and Psychotherapy programs.

Transportation and Dining

We offer transportation services for doctor appointments and for residents who need Dialysis, we use Ambustar. Most residents eat in their rooms. We do offer a dining room setting.

Reporting Concerns

The family and/or the resident needs to report any concerns and grievances immediately to the Nurse, Administrator or the Social Services Director. Depending on the complaint, need, concern or grievance the situation will be dealt with at once.

Rates

Our rates are: Private Room - $245/day (only 2 in the building); Semi-Private Room - $225/day; Ward Rooms - $220/day (4 residents to a room).

Communication

Communication with families and residents and among staff is of vital importance. Families are called regarding any concerns, med changes, accidents, sent out to ER, etc.

Rehabilitation and Respite Stays

We do offer Short Term Rehab, this is usually covered by insurance. I also offer Respite Stays. The cost is the same as above.

Pet Policy

We are a Pet friendly facility. Pets are allowed to visit their loved one. The pet must be up to date with their vaccines and we must retain a copy. Pets are not allowed during meal times.

Vaccinations

We offer Flu vaccinations as well as Pneumonia vaccinations to residents. We no longer offer Covid vaccinations.

Our Facility

We are a Skilled Nursing Facility, having said that we offer the Highest Level of Care, beyond our facility there is no other level to go. We are not a CCRC, Continuum of Care Residential Community. Currently, I do not have a waiting list.
