Room and Board

Security Deposit: $3,000

The room and board cost is per month:

  • Small Studio: $3,000
  • Medium Studio: $3,300
  • Large Studio: $3,500
  • One Bedroom Suite: $3,800

*2nd Occupant room and board fee is $1,600 per month.

Overview of Staff and Training

We have various types of professionals on our staff, including Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and Resident Assistants (RAs). RNs and LPNs are college-educated and hold licenses from the state. CNAs complete a community college program or other certification programs and are certified by the state. RAs receive in-house training at each facility. All staff undergo annual training through our corporate office, Illuminus, which is based on best practices in long-term care and assigned according to job roles.

Resident Safety and Well-being

Linden Ridge is equipped with a nurse call system and has an RA on duty at all times, an RN on-site 40 hours per week, and on-call during evenings, weekends, and holidays. Linden Court also has a wander guard system, RA on duty at all times, and an RN available under the same conditions as Linden Ridge. The LindenGrove skilled facility is staffed with an RN or LPN and CNAs at all times, along with a Director of Nursing (RN) who is on campus 40 hours a week and on call during weekends and holidays.

Resources for Transitioning into the Community

We offer a variety of resources and support services to help residents transition into the community smoothly.

Transportation Services

We do not provide transportation services directly. However, we have a contract with a local company, Seniors on the Go, to offer transportation services at market prices.

Dining Experience

Residents enjoy three meals a day with set times: Breakfast at 8 a.m., Lunch at noon, and Dinner at 5 p.m. They receive menus in advance and can choose between a main dish or an alternate option. Our menus change seasonally, and an always-available menu caters to any meal without advanced notice. We also have resident food committees that meet regularly to provide feedback on the menu and food quality.

Complaint and Issue Resolution

Residents can file grievances or complaints either orally or in writing. The Campus Social Worker, in partnership with the Campus Administrator, investigates these concerns, aiming to resolve and communicate the results within five working days. There is also a Corporate Grievance Hotline and resources available through The Ombudsman and the Wisconsin Division of Health/Quality Assurance office.

Family Communication

Upon admission, we collect a resident contact list to determine who is allowed to receive medical information. Residents have the final say on family contact. Monthly newsletters are sent via email from Linden Ridge and Linden Court’s receptionists. Staff are available to answer any questions or concerns families might have, and they can reach out to the nurse or Campus Social Worker at any time.

Short-term or Respite Stays

We offer respite care at all levels, subject to availability. The cost is calculated based on the monthly rate, divided by 30, and then multiplied by the number of days of respite. For example, a 14-day respite stay in our memory care home at an enhanced level of care would cost $3416. Payment is due before or on the date of admission. The level of care is determined by a nursing assessment for both Linden Ridge and Linden Court. The rate for Linden Grove skilled nursing facility is $490 per day.

Pet-Friendly Policy

We are a pet-friendly community. Pets must be up-to-date on vaccinations with proof required. Dogs must be leashed, and other pets should be in appropriate carriers. Residents at Linden Ridge can keep pets with them, given they can care for the pet themselves and provide a $600 pet security deposit along with a completed temperament questionnaire.

Vaccination Availability

We offer flu, COVID-19, and pneumonia vaccines to our residents.

Aging in Place

We support residents in aging in place and can add hospice services as needed. If residents at Linden Ridge or Linden Court require more services than we can provide, we facilitate transitions to our nursing facility.

Linden Ridge Assisted Living Apartments

Our campus features assisted living apartments, a memory care home, rehab, and a skilled nursing facility. I have availability in both assisted apartments and in our memory care home.

The assisted living has a large studio available for immediate occupancy and another large studio and a medium studio that will be available in the near future. There is a waitlist for one-bedroom apartments at Linden Ridge. We have a couple of available memory care rooms as well.

Our fee schedule for Linden Ridge is attached. The level of care is determined by a nursing assessment based on the care needs of the resident. Level of care is determined by room and board fee, based on apartment size, plus the level of care for nursing services, plus the level of care for personal services. The pricing sheet is attached to this email.

Linden Court Memory Care Home

We also have a memory care group home on the campus. This area is designed for residents with dementia, other cognitive conditions, or chronic conditions accompanying advanced age. The memory care tenants have a furnished, private bedroom with an attached bathroom, secure residence. Meals, medication, recreation, and daily nursing care included. Level of care pricing is attached to this email. Level of care is determined by a nursing assessment based on the care needs of the resident.

Lindengrove Skilled Nursing Facility

Provides subacute rehab services for patients needing rehab with plans to return to the community. Medicare or insurance only pays for rehab if skilled nursing or therapy criteria are met. Often, this is a short stay. Private rates in our skilled nursing facility are $490/day for room and board, plus therapy copay is typically $40-50 per session and part D co-pays for prescriptions.
