Overview of Professionals on Staff and Their Training
We have a skilled team of physical, occupational, and speech therapists who evaluate the rehabilitation needs of each patient and design an outcome-oriented treatment program. We are also staffed with RNs, LPNs, and CNAs around the clock to ensure that the most optimum quality of care is provided. Additionally, we have a full-time Registered Dietician, certified wound care nurses, and full-time Social Workers to ensure that all additional support needs are met. Each staff member goes through orientation and, depending on their role, trains with another individual. Continuing education courses are required for each staff member based on their role.
Measures to Ensure Safety and Well-being of Residents
We have 24-hour security and fire safety systems in place. Our building is a locked facility to ensure the safety of our residents. Each month, our team reviews quality measures to ensure that the needs and care of our residents are met. We also have a robust activities program that helps with the well-being of our residents. Individualized treatment plans are created for each resident.
Resources and Support Services for Transition into the Community
Yes, we have social services available to aid residents' transition into the community.
Transportation Services
Transportation to appointments is available for residents.
Dining Experience
Residents have the option to eat in our dining room or be served in their room. The menu is posted and updated each day throughout the facility. If the resident doesn't like what is being served, they are welcome to order off of the additional choice menu.
Process for Addressing Complaints or Issues
We hold resident council meetings where issues can be addressed. We also have a question, comment, and concerns form that residents can fill out if there is an issue. Our team member will ensure it gets to the most appropriate person based on the concern.
Policy on Communication with Resident's Families
It is important to us that families are involved in the care of their loved ones, provided that it is their wish and in compliance with HIPAA regulations. Each resident's profile also lists emergency contact(s).
Short-term or Respite Stays
Yes, we offer short-term and respite stays. The cost varies depending on insurance/payor. I can provide the contact information for our Business Office Manager if you are interested.
Pet-Friendly Community
Pet visitors are allowed as long as we receive a copy of their vaccines and they are all up to date.
