Overview of Services

At this time, we do have about a 3-5 year waitlist for long-term care but do also offer short-term rehab, pending bed availability. We have a full clinical staff including NP's, nurses, dietician, physical therapist, occupational therapist, speech therapist, and our physicians round a few days a week. All clinicians' complete their annual trainings and are up to date on all licenses. We assess all patients that are admitted to ensure any safety measures needed are in place, such as equipment needs and daily huddles to relay important patient information. We also have call lights attached to every bed for patient needs. Our infection disease nurse also provides any vaccinations residents may want including COVID and flu shots.

Discharge Planning

Once a patient is ready for discharge from our facility our case manager and social worker will work with the family for any needs. This includes home health, equipment, education, or community support groups. We do also provide transportation for any follow up care needed, dialysis, or doctor appointments.

Dining and Amenities

We offer 2-3 meal selections to our residents depending on the day and meals. We also allow residents to have personal snack items labeled in their rooms. We do also allow pets at our facility.

Emergency Communication and Feedback

Life Care Center does have an emergency communication system in place in case of any building emergencies or natural disasters in which we call families to provide updates and what plans are in place. We also encourage any concerns/feedback from residents and families. We have "blue cards" located throughout the building we ask to be completed for our social worker to address. We also have a monthly resident council meeting for our residents to bring concerns to staff attention.

Financial Information

We do not currently have any respite beds but short-term beds are based on availability at that time. Normally we bill insurance for short-term rehab but out-of-pocket costs normally run $442 daily. Currently, we do have a wait list for long-term care that is about a year long.
