LTC Placement Acceptance

I am accepting LTC placement.

Private Pay Rate

For private pay, our rate is $260.00 per day, $8,720.00 per month.

Overview of Staff and Training

In our facility, we have an Executive Director, Director of Nursing, Social Services, Business Office, Infection Preventionist/Wound Care Nurse, Dietitian, Certified Medication Aides, Certified Nursing Assistant, Licensed Nurses, and Registered Nurses who are all certified and licensed to provide excellent care to our patients. Every staff member in our facility undergoes training for safe patient transfers, fire safety drills, patient elopement drills, practicing adequate handwashing, COVID precautions, and customer service training. All of our staff are CPR/First Aid certified and have dementia training certifications.

Measures for Residents' Safety and Well-being

Our facility is locked 24/7, and the Front Desk monitors who goes in and out of our facility. We have a secured unit for our dementia/Alzheimer's patients to ensure they do not wander away from their familiar setting. All staff are required to pass a background check and drug screening before hiring.

Support Services for Residents' Transition

Our Social Services department coordinates discharges and works closely with New Mexico Aging and Long-Term Services to assist residents and their families who want to transition back into the community.

Transportation Services

Life Care offers transportation to and from residents' appointments.

Dining Experience

Life Care offers specialized diets based on medical condition, preferences, allergies, etc. Residents have an "Always Available" menu they can choose from if they do not like what is being served that day. Menus are rotated every 6 weeks.

Complaint and Issue Resolution Process

Life Care has a comment and concern card that is given to the designated department supervisor, who follows up and investigates the complaint/concern. It is then given to the Executive Director, who ensures that the complaint/concern is addressed.

Communication with Residents' Families

When a resident admits into our facility, the facility social worker has 24-48 hours to set up a care plan meeting with the family and patient to discuss the resident's care while in the facility. Another care plan meeting will be scheduled 10-14 days after the resident has admitted. If the resident stays for Long-Term Care, care plan meetings will be held every 3 months with family/resident to get updates on the resident's well-being. Any changes in medication or significant changes in the resident's health or well-being will be communicated to the family immediately.

Short-Term or Respite Stays

Life Care offers short-term and respite services. The cost for services for 30 days is $8,719.99 for a semi-private room and $10,061.67 for a private room.

Pet Policy

Pets are welcome to visit if vaccinated but are not allowed to stay with patients in our facility.

Vaccinations

Life Care offers COVID, Flu, and Pneumo vaccinations in-house. Family members are also welcome to take their loved ones out of the facility to get the vaccinations.

Aging in Place

Life Care has many residents who have aged comfortably in our facility. We do have a waiting list for our secured unit/dementia unit.
