Libertyville Manor offers six levels of healthcare. Our review committee will determine the appropriate category of care for resident's based on their medical and personal care needs. (The listed fees are based on a 30 day month):

Assisted Living (Licensed by Public Health):

Private Room, (1 person - half-bath) $134.00 per day / $4,020.00 per month.

Suite, (1 bedroom / full bath)...$148.00 per day / $4,440.00 per month

Suite, (1 bedroom, x-large dayroom / full bath / "1 person"). $208.00 per day / $6,240.00 per month

Suite, (1 bedroom, x-large dayroom / full bath / "2 person") $312.00 per day / $9,360.00 per month.

Light Intermediate Care:

Semi-Private Room $249.00 per day / $7,470.00 per month.

Private Room..... $288.00 per day / $8,640.00 per month.

Intermediate Care:

Semi-Private Room..... $288.00 per day / $8,640.50 per month.

Private Room..$355.00 per day / $10,650.00 per month.

Skilled Care:

Semi-Private Room $317.00 per day / $9,510.00 per month.

Private Room $400.00 per day / $12,000.00 per month.

Sub-Acute or Hospice Care - (end life < 90 days):

Semi-Private Room $399.00 per day / $11,970.00 per month.

Private Room...$450.00 per day / $13,500.00 per month.

LIBERTYVILLE MANOR DAILY RATES, INCLUDE THE FOLLOWING (ancillary charges/back page):

Twenty-four hour nursing care by licensed professional nursing staff.

Dispensing of all medications by a licensed professional, (no additional fee).

Comprehensive, individualized nursing and therapeutic assessment, with quarterly family care plan meetings.

Therapy evaluations by an "in-house" licensed professional therapist for safety or strengthening.

• Continuous communication between the facility, physician, and pharmacy related to all medical/health needs.
• Three balanced meals per day in a common dining area, or by bedside.
• Dietitian services, therapeutic / special diets, and snacks available upon request. (Guest trays available for nominal fee).
• Admission procedure, and maintenance of medical records.
• Processing of Medicare Part "A" / Part "B" claims, all operational documentation and activities.
• Social / Psychology services, consultation and rehabilitation. Discharge assistance with community services.
• Daily recreational programs, activity events, entertainment, crafts, and non-denominational spiritual services.
• Linen service including linens, towels & bedding (but not personal clothing). Free use of on site residential laundry facility. Availability of Great Room, lounges, private dining room, and patio at all times.
• Semi-Private, Private Rooms, and Suites available.
• All rooms comfortably furnished and equipped with a private half-bath (suites full bath), with wall to wall carpeting, individualized climate control, and television connection service.
• Housekeeping and maintenance services.
• On site inter-generational programs with Young At Heart Child Day Care!
