Our rehab section operates up to 6 days a week, with therapy hours up to 3 per day. We provide progressive, intense therapy for patients who need to return to the community and achieve a solid baseline of independence.

Types of Professionals on Our Staff and Their Training
We have a variety of professionals on our staff including RNs, LVNs, CANs, and Med Aides. Additionally, we have licensed therapy staff such as OTs, PTs, SLPs, and COTAs/PTAs. All our RNs have at least a bachelor's degree, LVNs have an associate degree, CANs and Med Aides hold certificates, and PT/OT/SLP/COTA/PTAs possess master’s degrees. All clinical staff members are required to undergo monthly training and weekly in-services to stay updated with the latest guidelines for long-term and rehab care and safety.Ensuring the Safety and Well-being of Residents
Our buildings take resident safety and well-being very seriously. Each patient has specific monitoring as part of their individual care plan. We adhere to QIPP measures and are accountable to our hospital partners, districts, and Health and Human Services. Yes, absolutely. We have licensed social workers who help coordinate care outside our facilities and assist with transitioning patients to their new homes.Transportation Services for Residents
Yes, we offer transportation services. The patient must be safe to be transported via wheelchair and meet safety guidelines. If not, a stretcher transport is facilitated through a private vendor to ensure safe assistance.Dining Experience
The food at this specific building is of quality, from taste to presentation. Although food can often be a complaint in facilities, Legend Oaks does not face this issue. The kitchen staff has been with us for several years and is familiar with the residents' preferences.Addressing Resident Complaints or Issues
Daily room rounds ensure each patient has an ambassador to discuss grievances with at any time. Staff are trained to inform their direct supervisor of complaints, leading to a formal grievance and investigation process.Policy on Communication with Residents' Families

Yes, we offer five-day respite stays with hospice. The price depends on the hospice contract. For short-term private pay without hospice, it is $215 per day for a semi-private room and $305 per day for a private room.Pet-Friendly Community

Yes, residents can receive Covid and flu vaccinations, as well as the pneumonia vaccine if they choose to.Aging in Place
Absolutely, residents are allowed to age in place, irrespective of whether they choose hospice services. There are no guidelines restricting their stay based on physical or mental decline, unless they pose a danger to themselves or others.
