Our assisted living start rates for accommodations are as follows:
- Studio apartments will begin at $4100
- 1 bedroom apartments will begin at $4850

Our Team and Services
We have an Executive Director, our new resident care coordinator will start on January 8th and another RN in the community. We have CNA’s scheduled 24 hours a day and one of our RN’s would be on call at all times. Several supporting departments, such as activities, dining, etc. We have ongoing training; in service once a month. All employees are required to have a food handlers, CPR and first aid training. We also have new staff orientation when they begin employment.
Security and Personalized Care
Our community doors lock at 7:30 PM, allowing us the opportunity to allow individuals into the community still, but with us knowing who is coming and going. Each resident who moves into the community are given a pendent for all emergency and non-emergency access, in addition to the pendent there is a plug cord in every bathroom. We have personalized care plans for residents with safety checks. Residents are given a key to their apartment so they can secure their belongings.
Welcome and Integration
We offer support to our new residents with the help of a welcoming committee. This welcoming committee comprises of current residents who live in the community, they have a little gift to give to the new resident and also does a welcoming lunch with them.
Transportation and Activities
We offer transportation for medical appointments in the Farmington area along with scheduled activities to local shopping centers.
Dining Experience
During dining we have a scheduled menu in addition to alternative options. Dining is all restaurant dining, meaning we have waiters and waitress who come to the residents tables to take their drink and food orders.
Resident Council and Feedback
We have a resident council meeting once a month. At anytime a resident can speak to our executive director, we send out annual surveys and have an anonymous suggestion box.
Communication and Family Involvement
When a resident moves into our community they are given a contact sheet with our leadership contact information and welcome to call at any time. Included on that contact sheet is our Medtech’s cell phone you can call at any time. If we ever have a situation with a resident or need to contact the family we will. If for any reason family feels we need to have a family meeting with our nursing department the family can request one at any time. This communication goes both ways, if we notice any care concerns we will reach out to the family and request a meeting.
Respite Stays and Pet Policy
We offer respite stays if we have availability in the community. For an assisted living respite stay it is $150 a day for Memory care it is $200 a day. We are pet friendly and allow small pets in the community, but do request shot records.
Health and Wellness
We usually schedule a day during the fall season were we offer flu vaccinations here in our community. Residents are able to age in place. We have 5 levels of care.
About Legacy House of Park Lane
Legacy House of Park Lane offers assisted living and memory care services in Davis County. We are located directly off the freeway on the north side of Cabela’s. We offer 133 apartments - 103 assisted living and 30 memory care. We offer upgraded apartments as well as patio or balcony options based upon availability. Outside Services Available Onsite
We have outside services available onsite to benefit our residents. We have a group called AMG that offers primary care services in the comfort of your apartment. Jenn Griswold, AGNP-BC, can manage medications, order labs, order mobile x-rays, EKG, offer physical exams, administer vaccinations, provide health screenings, manage chronic disease and work with you on your medical goals. This service is billed through your insurance. Also, every 6-8 weeks we offer podiatry care onsite. This is also billed through your insurance. In addition to that, we allow Home Health and Hospice agencies in our community to provide you with additional support during your stay. Right now we have a waitlist for our 1 & 2 bedroom apartments. Currently we have studio apartments available.
