Do you guys currently have a waitlist?

We tend to have a "waitlist" if you are looking for a long-term care bed. However, there are times where we can admit someone to our facility, but they would not be on our long-term care unit until a bed became available.

Could you share pricing and any other general information?

In the nursing home world, pricing varies based on the needs of the resident and those needs are determined by the Minimum Data Set (MDS) that is completed by our clinical team. We do accept Medical Assistance through the county.

Could you provide an overview of the types of professionals on your staff and the training they undergo at your facility?

All staff go through mandatory training at the time of hire based on the laws that are set in place by the Federal Government. This training is also completed annually. All departments have their department specific training that is completed with them at the time of hire. Our nursing staff go through an annual competency to refresh their skills.

We staff the following at our facility:

  • ADMINISTRATION: Administrator, Social Worker, Business Office Assistant
  • NURSING: Director of Nursing (DON-RN), Assistant Director of Nursing/Infection Preventionist (RN), RN Nurse Manager, LPN Care Coordinator, RN, LPN, Trained Medication Aide (TMA), Certified Nursing Assistants (CNA)
  • THERAPY (Select Therapy): Physical Therapist, Occupational Therapist, Speech Pathologist
  • CULINARY: Culinary Director, Culinary Cook, Culinary Aide
  • ACTIVITIES: Therapeutic Recreation Director, Therapeutic Recreation Aide
  • HOUSEKEEPING: Housekeeping Aides
  • LAUNDRY: Laundry Aides
  • MAINTENANCE: Maintenance Director, Maintenance Assistant

Could you tell me about the measures you have in place to ensure the safety and well-being of your residents?

We staff our facility with a licensed nurse on a 24/7 basis. There is on call providers through Mayo Mankato and Mankato Clinic that are available on call for our staff to contact on a 24/7 basis.

Do you offer any resources or support services to aid residents transition into the community?

Our social worker and the corresponding nurse manager for the resident would assist with the transition for the resident.

Do you offer transportation services for residents?

We do not have transportation services for our facility. We partner with the different transportation companies in the Mankato Area to set up rides for residents when needed. This cost would be the responsibility of the resident unless they are on a skilled Medicare stay.

Could you tell me a little about your dining experience?

We offer three meals per day and upon request for snacks, we have a water pass that occurs in the morning and evenings. Residents are encouraged to enjoy their meals in the dining room but can choose to stay in their rooms for mealtimes if they are deemed safe from a clinical standpoint.

What is the process that takes place if a resident has a complaint or an issue they want addressed?

If there is a complaint/grievance that arises, we have a policy and form that residents, families, visitors can fill out and turn in for our leadership team to investigate/follow up with.

What is your policy on communication with resident's families? How do you keep them updated about their loved one's well-being?

If the family is on the medical record for the resident, the family can feel free to call the facility and ask for an update. If a resident has a change in condition, has an accident/incident such as a fall, change in medication, etc. it is expected that the resident/family/responsible party for the resident is updated in this change.

Do you all offer short-term or respite stays? How much does this cost?

We do offer short term rehabilitation. The cost of this would be dependent on if the resident had the qualifying 3-night hospital admission prior to being admitted to our facility. If they had the qualifying 3-night hospital admission and have Medicare, Medicare would cover the stay along with any secondary insurance. If there is no 3-night hospital admission, then it would be considered private pay which the resident is responsible for - At the time of admission for private pay, we do ask for a $5,000 down payment at the time of admission.

Are you all a pet-friendly community?

Yes, we are for visits - we just ask that we have the copies of their vaccinations on hand.

Are residents able to get Covid vaccination and flu vaccinations in the community?

Yes, all residents are offered the vaccinations at time of admission.

Are residents able to age in place?

At Laurels Campus, we do have an assisted living facility attached to our nursing home. Residents are absolutely able to age in place which is a huge benefit for our residents at the Laurels Campus. We tend to have a "waitlist" if you are looking for a long-term care bed. However, there are times where we can admit someone to our facility, but they would not be on our long-term care unit until a bed became available.
