Staff Overview and Training

Could you provide an overview on the types of professionals on your staff and the training they undergo at your facility? Nurses, care providers, activity providers and Memory Care Coordinator, Chefs. All staff go through extensive Memory Care Training with the program that we have in place called MMIT (Montessori Moments in Time), monthly all-staff meetings, and training on Relias computer training.

Resident Safety and Well-being

Could you tell me about the measures you have in place to ensure the safety and well-being of your residents? We have monthly drills, in-services for training, extreme weather condition drills, offer two-hour checks if needed, allow all medical equipment for the well-being of the resident, and provide CPR and First Aid training.

Transition Support Services

Do you offer any resources or support services to aid residents' transition into the community? Total Life is a one-on-one service that is billed through medical insurance and is a Zoom call that we help the person with.

Transportation Services

Do you offer transportation services for residents? We offer transportation Tuesday and Thursday within a 25-hour radius for appointments, and additional fees apply for farther distances.

Dining Experience

Could you tell me a little about your dining experience? We offer an open-concept dining experience with meals being served in the dining room. We have tables set with linens, napkins, silverware, and glasses, and music playing during meals. Choices of entrees, dessert (always available), and offer 2 liquids per Meal. We have servers who know the choices and diets of the people they serve. We have an alternative menu if the choices offered are not desired. The Memory Unit has many programs in place and offers three snacks a day on top of the meals served, hand washing with scents to help stimulate appetite, color contrasting place settings, hydration, finger foods if needed, and staff on hand.

Complaint and Issue Process

What is the process that takes place if a resident has a complaint or an issue they want addressed? We have a Grievance Committee and a Resident Council meeting monthly where residents can voice concerns. We also have an open-door policy with the Administrator that anyone can take complaints to.

Cost of Living

What is the cost of living in your community? I have attached the price list for you for both Memory Care and Assisted Living. The cost is the room rate and the level of care needed (determined by the assessment completed by our Nursing Supervisor). Both the room rate and level of care is a monthly fee. There is a $2000 one-time entrance fee.

Communication with Families

What is your policy on communication with resident's families? How do you keep them updated about their loved ones' well-being? Any falls or medical changes are communicated to the family. We offer a care conference quarterly as well.

Short-term and Respite Stays

Do you offer short-term or respite stays? How much does this cost? We do offer respite stays; again, those prices are on the price sheet, and they are not charged the level of care fee.

Pet-friendly Policy

Are you a pet-friendly community? We are a pet-friendly community, and the resident does have to be able to care for and clean up after the pet and also have all shot records up to date. They also have to be on a leash if outside of their room.

Waitlist Status

Do you currently have a waitlist? We have just started on a waitlist at this time.

Vaccinations

Are residents able to get Covid vaccinations and flu vaccinations in the community? Yes, we have a pharmacy that comes in and offers those vaccinations.

Aging in Place

Are residents able to age in place? We do believe in aging in place.
