I have attached the brochure to answer many of the below questions. We offer Independent Living, Assisted Living, Long Term Care, Skilled Nursing, Rehab, Dialysis Unit, and Respite stays in our Long-Term Care.

Physician Teams and Campus Services

We offer all residents in Assisted Living and Long-Term Care/Skilled Nursing the option to see one of our Physician Teams that complete rounding here at the campus. We have all departments here at the campus. Many of our positions require degrees and license in the area of study below. We complete ongoing training with a software called Relias.

• Administration (Licensed Nursing Home Administrator and Operator’s License)
• Activities (Social Service Designee’s)
• Social Services (Two Licensed Social Workers)

Dining Options

In A.L., LTC and SNU all residents have two choices for breakfast, lunch and dinner and snacks. There are dining rooms that residents will attend for meals.

Pet Policy

Independent Living allows dogs and cats. Assisted Living only allows cats if the resident can fully care for the cat.

Safety and Assistance

We offer pendants and call lights in A.L., LTC and SNU for all residents to request assistance.

Grievance Procedure

If a person has a grievance, we offer a formal grievance option and that can be documented with the Administrator, Executive Director or the Social Workers. If it’s something that is beyond an on-site grievance you may file a complaint with the State Ombudsman’s Office.

We offer Independent Living, Assisted Living, Long Term Care, Skilled Nursing, a Dialysis Unit, and a Physical Therapy Department. We have a wide range of services to offer. I have attached a brochure, floor plans, the list of those with the Wesley Team and information on the Dialysis Unit.

Long Term Care stays which is also paid for with the new resident’s funds or thru Medicaid Title 19. You can reach out to your current Medicare Insurance and start the process of application for Medicaid Title 19 as well. Assisted Living stays which is paid for with the new resident’s funds or thru Medicare HCBS Frail and Elderly (65 years old +). We accept many traditional insurances including United Healthcare, Aetna and Sunflower. We also accept the Hope Program and Management Care Programs. The assisted living and independent living apartments do not come furnished.
