Overview of Staff and Training

We have LVN’s, med techs, and caregivers. All staff are required to have monthly education training. Our community also offers on-site outpatient physical therapy and occupational therapy through Empower Me. We have an in-house physician that makes rounds bi-weekly.

Safety and Well-being Measures

Each resident has a tailored care plan specific to their needs. We assess for fall risks and monitor any dietary restrictions. All residents receive a call pendant for emergencies. For those at risk, we also offer wanderguard.

Resources and Support Services for Transition

We have an engagement coordinator assigned to new residents to help them transition into the community. Resident ambassadors also help facilitate this transition. We have an Evergreen support group for families needing added support moving their loved one to Memory Care.

Transportation Services

Transportation is provided within a 10-mile radius, with doctor’s appointments taking priority.

Dining Experience

We offer restaurant-style dining, open from 7am to 7pm. We provide healthy choice items and follow any dietary or allergy restrictions. Residents order from the all-day menu and also have daily specials. Three meals each day are included. We offer diverse menu options that are on a five-week cycle.

Complaint and Issue Resolution Process

All residents can speak with our Executive Director to file a grievance and also have access to an Ombudsman.

Communication with Resident's Families

Our community is very family-friendly. During visits, if you wish to speak with a team member to see how your loved one is settling in, we’re always happy to offer our insight. All residents have an assessment to help us understand how we can support them daily. We also conduct a second assessment after 30 days to see if care needs to be added or removed.

Short-Term or Respite Stays

We offer short-term and respite stays. The process is the same for all our residents since we are a month-to-month community. There would be a $6,000 community fee that is 80% refundable if a resident moves out at 30 days. The cost depends on when the resident moves in. In assisted living, we have summer pricing for our one-bedroom apartments at $4,995. The normal market rate is $5,795.

Pet-Friendly Policy

We are pet-friendly. There is a nonrefundable deposit of $500. All pets must be up to date on their shots and registered.

Vaccinations

With a doctor’s order, residents can get vaccinations. We also offer annual clinics at our community.

Age in Place

Yes, our residents are able to age in place. We offer a variety of floor plans ranging from studios to two-bedroom, two-bath apartments. Depending on your preferred floor plan, there may be a waitlist.
