SILVERCARE SERVICES AND PRICING

Monthly care service fees for assisted living are based on the resident’s score from the Care Level Evaluation tool. A licensed nurse determines the resident’s level of care by assigning point values to the resident’s individual needs, including symptom management. Level of care evaluations are completed initially when determining the monthly costs. Re-evaluations will be done as needed when there is a change in status.

BASIC SERVICES

Daily reassurance check, Resident Assistant (RA) availability on-site 24/7, maintenance of health records, monthly vitals check, 90-day assessment by an RN. Basic Service fee is a required minimum monthly fee.

Overview on the Types of Professionals on Your Staff and the Training They Undergo

Our staff all go through fingerprinting, background, and drug testing. They are required to attend new employee orientation and take a significant number of online training courses related to dementia. We have CAN’s and RAs for our care staff along with our RNs and LPNs.

Measures to Ensure the Safety and Well-being of Residents

We provide quality care for all of our residents while offering activities to help with life enrichment. For residents on our care services, they receive “safety checks” at each scheduled service and/or a daily phone call. Independent residents are just that, independent. Our staff and residents are fabulous at notifying staff if someone hasn’t been seen or if they seem not themselves, and we conduct a wellness check. Safety is our number one priority!

Resources or Support Services to Aid Residents' Transition

I work with families throughout the entire move-in process. We also have ambassadors on each floor to welcome our new residents and invite them to either a meal or activity. All staff are available to assist with the transition and navigation.

Transportation Services for Residents

Yes, our shuttle is available for all of our field trips and outings. Each Wednesday morning at 10, the shuttle goes to the grocery store, rotating between Target, Cub, and Walmart. We also offer medical runs on Wednesday afternoons within 10 miles of our community. There are limited seats for all outings and offerings, so we have a sign-up sheet at our front desk.

Dining Experience

We currently offer all-day dining so residents simply come down and take a seat and they are greeted restaurant-style. There is one lunch and dinner entrée served daily, but if that item isn’t preferred, there is a menu of other all-day options. I often eat here myself, and the food is very good.

Process for Complaints or Issues

Residents are always encouraged to come to staff to share any issues. Typically, they are directed to the director of the specific apartment if that relates, or if not, then directly to our Executive Director.

Policy on Communication with Resident's Families

Each resident has an assigned designated representative who we would call in any emergent situations and email with all critical community information.

Short-Term or Respite Stays

We have a respite suite, if available based on occupancy. I have attached a flyer to easily show all the costs involved as well as what is included. Hope that will help.

Pet-Friendly Policy

We allow pets up to 35 lbs with a non-refundable deposit and monthly pet rent. All dogs are allowed to visit our community as long as they are good with people, etc.

Covid and Flu Vaccinations

Yes, unless that changes, we have always offered a clinic here in our activity center for COVID, boosters, flu, and sometimes even shingles.

Availability of Apartments

We currently have a small waitlist; however, most have been turning available apartments down for a while now. Currently, we have one open 1BR for assisted living.
