Overview of Professionals and Training

In our Rehab & Care Center, we have a diverse team of healthcare professionals including two in-house physicians, 3 Registered Nurses, and 5 Certified Nursing Assistants on each station for every shift. Our contracted partnership with Infinity Rehab provides physical, occupational, and speech therapy as needed. Our staff also includes a social services team, a dietary team, an activities team, and an environmental team. New staff members undergo comprehensive competency training, supervised by experienced department supervisors.

Safety and Well-being Measures

We prioritize the safety and well-being of our residents with 24/7 nursing staff, an emergency call system, and a front desk staff monitoring facility access. Our environmental team ensures the maintenance of equipment and facilities, and we have robust emergency procedures. An Infection Control Coordinator implements CDC policies and oversees infection prevention, including COVID-19 containment measures. We also have isolation rooms for residents requiring quarantine.

Transition Resources and Support Services

Our social services team facilitates safe discharges to homes or supportive housing facilities, coordinating with physicians and making necessary referrals. We assess each resident's needs and preferences, often involving family in the planning process. Our nursing and therapy teams provide tailored recommendations for post-discharge care. Placement in our facilities is based on individual needs and availability, without a waiting list.
