Overview of Professional Staff and Training

Our professional staff includes Certified Nursing Assistants (CNAs), Licensed Practical Nurses (LPNs), Registered Nurses (RNs), and a Dietitian. All employees are credentialed, ensuring they meet state-required training, certification, and/or licensing requirements. We contract our therapy staff and our Medical Director, Dr. Michael O'Conner of UnityPoint, who, along with his team, is on-call 24-hours per day and makes weekly visits to our facility.

Transportation Services

Transportation is available most days of the week. Round trips cost $110, comparable to other wheelchair van services in the Des Moines area.

Dining Experience

Meals are served three times a day: breakfast at 0730, lunch at 1130, and dinner at 1730. Each meal features a main entree, with a substitution list available for those who prefer an alternative.

Resident Complaint and Issue Resolution

Residents can report issues at the monthly resident council meeting or to a charge nurse, the Director of Nursing (DON), the Administrator, or the Grievance Officer. Issues are then discussed in a manager meeting to propose a solution.

Cost of Living

The cost is $235/day for a semi-private room and $270/day for a private room, with rates expected to increase within the next month.

Communication with Residents' Families

We notify resident representatives of medical changes, new doctor orders, etc., as required by regulation. Care conferences are held quarterly at a minimum to discuss the resident's care plan.

Short-term or Respite Stays

We offer short-term or respite stays at the same rates mentioned above, requiring a minimum stay of 7 days.

Pet-Friendly Community

Pets are welcome to visit, provided they are on a leash, up-to-date on vaccinations, and do not exhibit aggressive behaviors.

Covid and Flu Vaccinations

Residents are able to receive Covid and flu vaccinations within the community.

Aging in Place

Residents can age in place as long as their needs are met and they do not pose a threat to themselves or others. We currently have a waitlist for Medicaid and immediate openings for private pay or skilled rehab for both a female and a male resident.
