Assisted Living Memory Care

Assisted Living Service Fee

In addition to the monthly rental fee, a monthly assisted living services fee is required based on the amount of care provided.

A nurse will meet with you to complete an initial assessment and make a recommendation of services that could be provided to meet your needs. The services provided will correspond to one of three package levels. These are summarized below with examples of services offered at each level. Personal Care Level I: $3,055/month

  • Prompts and reminders for grooming (no physical assistance)
  • Bathroom assistance (1–3 times daily)
  • Shower assistance (1–3 times weekly)
  • Minimal behavior redirection
  • Dining assistance for snacks, beverages, and light breakfast meals
  • Simple medication management (1–14 meds), providing set-up and administration (1–4 times daily); does not include injections

Personal Care Level II: $4,170/month

  • Simple or standard dressing and grooming assistance (no transfer assistance)
  • Standard bathroom assistance (4–5 times daily)
  • Shower assistance (4–5 times weekly)
  • Dining assistance with plate set up and preparation to eat
  • Medication management (15+ meds) and administration (5–6 times daily); includes blood sugar monitoring but not including injections
  • Oxygen management
  • Moderate behavior redirection
  • 1-person transfers

Personal Care Level III: $5,285/month

  • Extensive dressing and grooming assistance (including transfer assistance)
  • Extensive bathroom assistance (6+ times daily)
  • Shower assistance (6+ times weekly)
  • Dining assistance including plate set up and observation, cueing to eat and feeding
  • Medication management and administration (7+ times daily); includes blood sugar monitoring, insulin injections, and sliding scale management
  • Oxygen management
  • Complex or frequent behavior redirection
  • Mechanical lift transfers

Included in Your Assisted Living Service Fee:

  • Laundry (up to 3 loads per week)
  • Escort to meals and activities
  • Assistance with ted hose, ace wraps or Velcro leg wraps
  • Daily reassurance check

Overview of Our Staff and Training

We have a variety of professionals that work in our community, including nursing, resident services, culinary, maintenance, administrative, receptionist, and life enrichment staff. Each position has specific requirements for the role. Once hired for PHS, we have onsite training tailored to the specific position.

Ensuring Resident Safety and Well-Being

We regularly review fire and weather drills. In assisted living, we conduct daily checks on all residents. In assisted living memory care, we check on residents every 2 hours.

Support Services for Transitioning Residents

We offer complimentary escorts for the first week to help new residents get acclimated to the community. Staff and other residents, designated as β€œambassadors,” partner with new residents to help them meet others and support them through the transition.

Transportation Services

We provide a shuttle service that can transport residents.

Dining Experience

We have three dining options:
  • A dining room on the 3rd floor in the assisted living area offering lunch and dinner
  • A bistro where food can be ordered
  • A restaurant where residents can dine

Handling Resident Complaints

Residents are encouraged to bring concerns forward to anyone they feel comfortable with. For anonymous submissions, we have a designated place for comments and concerns. Once a complaint is received, we investigate and address it as appropriate.

Communication with Residents' Families

We communicate with families if the resident provides permission. A designated responsible party will be updated on the resident's well-being.

Short-Term or Respite Stays

We do not offer short-term or respite stays in assisted living. However, our care center provides short-term rehab, and costs depend on the resident's needs.

Pet-Friendly Community

Yes, we are pet-friendly according to our pet policy. Cats and dogs must be 20 lbs or less, and residents must be able to care for their pets independently.

Waitlist Information

The waitlist status depends on the specific area you are looking to live in. Details were emailed to you in the first email.

Availability of Vaccinations

We offer flu vaccination clinics. We no longer offer COVID vaccination clinics since the vaccine is readily available in the community.

Aging in Place

Residents can age in place within our community by moving through the continuum of care levels as needed. However, they do not stay in the same apartment but move as required by their care needs.
