LEVEL 1 Friendly reminders to ensure regular, healthy habits are maintained, not overlooked. Supervision and hands-on assistance are not offered at this level of care. $600.00 Per Month

LEVEL 2 Guidance, encouragement and hands-on assistance, as needed, for residents who benefit from reminders and oversight as they self-manage their tasks of daily living. $1,000 Per Month

LEVEL 3 A plan with additional caregiver time for help with resident-directed daily living activities-including, toileting routines, personalized bathing preferences, or verbal cueing $1,600 Per Month

ENHANCED CARE OFFERINGS

In order to fulfill our mission to serve older adults and their families as they age, Cedarhurst also offers an enhanced level of care for our residents should it ever be needed. Cedarhurst can also coordinate additional services on-site, helping to ensure your loved one doesn't have to experience the disruption of a move.

LEVEL 4 Hands-on, individualized care for residents with diagnoses such as Parkinson's or arthritis, or with balance issues that result in decreased mobility and functional inabilities. $2,200 Per Month

LEVEL 5 A personalized services suite provided by one or two caregivers for residents with combinations of mobility, balance or motor challenges, or cognitive impairment. $2,800 Per Month

Could you provide an overview on the types of professionals on your staff and the training they undergo at your facility?

Our Caregivers do not have to be CNAs but some of them are. They are CBRF Trained which is a training mandatory for the state of Wisconsin. We do have training with our staff when hired and updated monthly on certain criteria. We have a nurse in the building for most days and on call 24/7.

Could you tell me about the measures you have in place to ensure the safety and well-being of your residents?

We have an emergency response pendant system that we use here. The front entry is staffed from 8am-5pm to ensure the residents have a place to come for questions. We have a cell phone that rings after hours. The doors are locked at night to ensure the safety of the residents. We have a nurse on call 24/7. There are pullcords in all the restrooms that go directly to the caregiver’s pagers.

Do you offer any resources or support services to aid residents' transition into the community?

Yes! We have a wonderful resident ambassador who sits with and helps the new residents feel comfortable. She will go to activities with them and introduce them to the residents. We also have monthly ice cream social parties where we ask new residents to come and meet everyone. When they move in all managers go up and introduce themselves and make sure the resident has all of our contact information.

Do you offer transportation services for residents?

They are currently working on setting up a couple days that we will take residents to the doctor within a certain mileage. I am unsure of the exact details as this is in the works.

Could you tell me a little about your dining experience?

Sure, We build our menus based on individual dietary restrictions, customizations, and requests, with all-around nourishment and deliciousness that’s hard to come by anywhere else. We are very excited to be having “choices” on the menu and we are currently working hard to implement this.

What is the process that takes place if a resident has a complaint or an issue they want addressed?

The resident or family member would go to the executive director with any complaints or issues. If it is a nursing concern, we would have you/them meet with the nurse so we can try and fix the problem.

What is the cost of living in your community?

Cost is depending on the size of apartment and the care level of the resident. We have studios, one-bedroom and two-bedroom apartments available. We also have several different levels of care depending on an assessment the nurse would complete free of charge.

What is your policy on communication with resident's families? How do you keep them updated about their loved ones' well-being?

If the resident has an activated power of attorney, we would absolutely keep them updates. If the resident requests, we contact the family we will do that also. We have a Facebook page, a texting system, and a newsletter that is also full of information and photos from the activities. Families are always welcome to call management with any questions or concerns they have about their loved ones.

Do you all offer short-term or respite stays? How much does this cost?

Yes we do offer respite with a minimum of thirty-day stay. The cost is $225/daily.

Are you all a pet-friendly community?

Yes absolutely! There is a fee but pets are welcome at charter. We love pets 😊

Do you guys currently have a waitlist?

There is not a current waitlist at this time. We do have a few open apartments that face the lake open currently.

Are residents able to get Covid vaccination and flu vaccinations in the community?

Yes, they have a nurse that comes in yearly, and they can sign up for the shots if they want.

Are residents able to age in place?

Absolutely, We have enhanced levels of care so the residents are able to age in place.
