Overview of Professional Staff and Training

We have a mixture of RNs, LPNs, and CNAs present 24/7. They undergo extensive training before joining our staff and participate in several days of orientation to become familiar with the facility before beginning their duties on the floor. In addition, we have a medical director who visits weekly to see the residents, while his nurse practitioners check in throughout the week. A member of his team is always on call for emergencies.

Safety and Well-being Measures

We practice person-centered care here at Hooverwood. Each resident has a personalized care plan that we follow and adjust as changes occur in their medical care.

Support Services for Community Transition

We have several social services employees available to assist. They will schedule a care plan meeting within 72 hours of a resident's admission to the skilled side of the community. They address any concerns with the resident and family, with care plan meetings scheduled quarterly or upon request.

Transportation Services

Yes, we provide transportation services Monday-Friday, pending availability.

Dining Experience

We offer three meals a day and provide snacks throughout the day upon request. Our menu includes both set and always available options, allowing residents to choose meals they prefer. Staff members ask each resident about their meal preference before each meal.

Complaint or Issue Resolution Process

Residents are encouraged to speak with their unit manager or social worker should any issues arise, as they will know how to address the concerns.

Communication with Resident's Families

We maintain a list of emergency contacts for each resident. If there's a change in a resident's status, we call down the list until we reach someone. Upon reaching the first contact, we trust they will inform the family. Care plan meetings further ensure communication of their loved ones' care.

Short-term or Respite Stays

We offer rehabilitation, long-term care, memory care, respite, assisted living, and hospice services. Respite care requires a minimum stay of one week. Medicaid can cover room and board, otherwise, it's a private pay arrangement. Private rooms in long-term care cost $445/day, with full payment required before admission. Respite stays are also available in our assisted living community, with a minimum of 30 days required.

Pet Policy

We allow visiting pets, provided we have vaccination records. Residents in assisted living who can care for their pet independently may own pets.

Covid and Flu Vaccinations

Yes, we offer Covid and flu vaccinations within the community, especially during respiratory season.

Aging in Place

Our goal is for each resident to age in place, prioritizing their safety. We make recommendations if relocation within the facility becomes necessary. We accept applications for the waiting list but currently have a delay due to renovations, particularly in our memory care unit. We prioritize transparency to aid residents in making informed decisions.
