Rehab Cottage ~ Post-acute Care

Post-acute rehabilitation is typically covered by Medicare for the first 20 days when the patient has stayed overnight in the hospital for at least three consecutive nights (status must be “inpatient” not “observation”). For days 21 to 100, a co-pay applies that is set by Medicare. For Medicare Advantage plans, consult your plan benefits summary.

Overview of Staff and Training

Our staff includes RNs, LPNs, CNAs, Medication Aides, as well as other ancillary service employees. In addition to any certificate or license that a team member might hold, we conduct regular skills labs and training sessions on various relevant topics related to current best practices and common disease processes.

Measures to Ensure Safety and Well-Being of Residents

All of our cottages are secure, with a keypad entrance and exit. The code for the keypad is not shared with anyone not on staff. Team members let visitors into the cottage to maintain awareness of who is visiting. We have protocols for falls, conduct regular rounding on our elders, and have other safety measures in place. We develop an individualized plan of care for each elder to best meet their needs and keep them safe.

Support Services for Transition into the Community

We gather information about a resident’s social history and preferences before admission and share that with team members. We assist residents in getting familiar with and acclimated to the community as needed and check on them regularly to ensure they are involved in desired activities and to answer any questions. We also have access to other service lines within the Hillcrest family for additional support.

Transportation Services

We offer transportation services with a van and a bus, and a chauffeur is available to provide transport. If she is already booked when transportation is needed, she will either schedule with an outside provider or reschedule the appointment based on the preference of the elder and their family.

Dining Experience

Each cottage has a large, open kitchen and a dining room. We encourage elders to dine together in the dining room. If they are unwell or prefer to stay in their suite, meals will be delivered to their room at no additional charge. Breakfast is cooked to order, and there are two entrees available for lunch and dinner, with an always-available menu if neither option is preferred.

Addressing Complaints and Issues

We encourage elders and families to discuss any concerns with our team. Generally, concerns are reported to our social services representative, but any team member can convey concerns. We aim to resolve issues quickly and have a follow-up process managed by our social services representative and overseen by the administrator.

Communication with Residents' Families

We update families regularly on changes in condition, falls, medication changes, etc. Regular quarterly life plan meetings are held, and participation is encouraged. Families and elders can also talk with the cottage nurse or a member of the administrative team. Information is shared with designated individuals, and we send monthly newsletters and email updates as appropriate.

Short-Term or Respite Stays

We offer short-term stays with a minimum of 30 days. Costs are based on our regular daily rates, supplies needed, transportation, etc.

Pet-Friendly Policy

Pets can visit the cottages after providing current veterinary records. Pets must be leashed or in a carrier in common areas but can be off-leash or out of the carrier in the suite with the door closed.

We currently have limited availability at the cottages.

Covid and Flu Vaccinations

We assist our elders in getting COVID and flu vaccinations at the cottages.

Aging in Place

The cottages are a skilled nursing facility level of care and can manage most needs through end of life with only a couple of exceptions. We currently have one private suite available in long-term care and a companion suite for a lady.
