Overview of Staff Professionals and Training

HW provides 24-hour Certified Nursing Assistance care with related certifications and requirements. The staff includes Licensed Practical Nurses (LPNs) and a Director of Nursing who is a Registered Nurse (RN). The RN is on staff and in the building during business hours and on call when away from the community.

Safety and Well-Being Measures

Front doors are unlocked during business hours from 9 am to 5 pm, and each resident has their own set of keys to the community, their apartment, and mailbox. Residents are required to sign themselves in and out when leaving the community. Well-being checks are conducted at mealtimes, and medication assists are provided.

Support Services for Transition

Transition aids include friendly reminders and invitations to activities. The staff, including HW Ambassadors and the Resident Services Coordinator, engages with new residents to integrate them into the community and make them feel welcome.

Transportation Services

HW assists residents in scheduling transportation with North Central Area Transportation (NCAT) or Mendota Area Senior Services (MASS). A HW bus is available for shopping and pleasure trips.

Dining Experience

Residents enjoy restaurant-style dining for three meals daily. Breakfast is served from 7-9 am, and lunch and dinner have two seating times at 11:30 am/12:30 pm and 4:30 pm/5:30 pm, respectively. The dining experience includes table service, with drinks and meal orders taken by staff. Residents are encouraged to attend at least two of the three daily meals.

Complaint and Issue Resolution Process

Complaints or issues can be addressed in person to a manager, the Executive Director, the Nurse, or another staff member. There is a suggestion box and an Ombudsman who visits monthly. The Care Merge App also allows families to stay connected and address concerns.

Family Communication Policy

HW complies with HIPAA and maintains an open door policy. Families can use the Care Merge App to stay updated and receive email notices from the Executive Director. They are also encouraged to visit and participate in activities and special events.

Short-Term and Respite Stays

HW offers a month-to-month lease with a 30-day written notice for discharge or move-out. All admission procedures are followed and the pricing remains consistent.

Pet-Friendly Community

HW is a pet-friendly community, provided the resident can take care of the pet including vet visits, vaccinations, food, and toileting. Yearly documentation for the pet is required.

Waitlist Information

HW is actively admitting residents as their files are being approved, and currently has a short waitlist that is being managed.

Vaccinations

HW has held flu and COVID-19 clinics on-site in the past and evaluates the need to host such clinics each year with a participating pharmacy.

Aging in Place

Aging in place is a goal at HW. Each resident is assessed for health, safety, dignity, and compassion, and personalized care plans are in place to support their aging process.
