Front Line Staff
As an Assisted Lifestyle Community β Supportive Living we have the following front-line staff:
- CNAs β 24hr staff, certification as required by the State of Illinois
- Nursing staff β Work a regular weekday schedule, on call for assisting CNAs after hours and weekends (Director of Nursing and two LPNs) based on certification by the state
- 2 full-time activity staff
- Dietary Aides, cooks, for services in our dining room
- 4 housekeepers and 2 maintenance staff.
All staff are regularly required to participate in educational training programs. CPR is required by all CNA and Nursing staff and offered to all staff.
Assisted Living Safety Measures
We provide many services to assist residents in their daily living activities. We have an alert system throughout the building for residents to request help. All units are accessible. We follow seven dimensions of wellness to address various aspects of wellness from physical (fitness classes, healthy eating) to social activities.
Resident Services
Our Resident Services Coordinator conducts a new resident survey, and residents are given a welcome packet of information. We encourage participation and try to pair up neighbors to help acclimate new residents.
Bus Services
Our Bus is available for transportation to the Greater Rockford area (outside Boone County) on Tuesday for medical appointments. For bus service inside Boone County, accessible bus service is offered through the Boone County Council on Aging. We also have a weekly outing to a variety of places.
Dining Services
Our dining room serves three meals a day in a restaurant style (menu/servers). Breakfast is served from 7am-9am, Lunch from 11am-1pm and dinner from 4pm-6pm. Residents do not have to come at any specific time but need to present for meals at least 15 minutes before the end of each service so we have time to serve them. We do not offer room service. Sick trays are approved by nursing if needed and consist of ginger ale, broth, Jello, and crackers.
Resident Council and Meetings
Each month there is a resident council meeting, run by the residents. The director attends only by invitation. This is an opportunity for residents to air concerns and ask questions. We also have a monthly food committee meeting. The Executive Director is also available to connect with directly. For maintenance requests, the resident calls or goes to the front desk to have a work order completed.
Rates and Fees
We are both a self-pay and Medicaid-supported community. I have attached our current rate and fee sheet.
Family Engagement
Caremerge is our family engagement and communication system. Notices and communications are sent to approved family members via this system.
Short Term or Respite Care
We do not offer short-term or respite care. Our resident lease is one year. We do not have any community fees, application fees, deposits, or buy-in costs.
Pet Policy
We are pet-friendly. Cats or dogs (up to 50lbs) incur a $250 nonrefundable fee. Veterinary records and a pet assessment are required. A responsible party must be selected in the event a resident is out of the community, and the resident must also show their ability to care for the animal.
Vaccination Clinics
We generally have vaccination clinics, depending on the time of year and availability of vaccinations. Yes, when offered, residents can sign up for these in the community.
Aging in Place
If a person exceeds our level of care, we would work with the family to move that resident to the needed level of care. For example, if a person needed a locked memory care facility, this would no longer be an appropriate community. Many people are able to be with us their whole lives. While there are services we can provide for a personβs daily needs, if skilled care is needed, that would also be a reason to move to a higher level of care.

We currently have a waiting list. For an individual, I would estimate about four months.
