Level of Care Information
Hayes Manor has 3 different tier Levels of Care. These levels of care have been designed so our residents can “Age in Place”. Once a potential resident has been accepted, the resident, family, and care takers can relax with the comfortability of knowing, they will be taken care of even if their medical diagnoses increase.
Level 1: Primary Care
Fee is $500 additional per month. Additional Services included:
- General Services
- Assistance with bathing, grooming, and dressing
- Occasional reality orientation
- Reminders with self-managed incontinence and assistance when needed
- Daily Safety Checks
- General monitoring and supervision.
Level 2 Extended Care
Fee is $850 additional per month. Additional Service Included:
- General Services
- Level 1 Primary Care
- Assistance in coming to meals
- Weekly shopping expeditions with accompaniment if necessary.
- Assistance making appointments and getting to the appointments.
- Frequent reality orientation
- Assistance with bathing, grooming, and dressing.
- Continence management assistance
- Frequent safety checks
- Extended counseling provided by our Nursing staff.
Level 3 Extended Care and Special Needs
Fee is $1,200 additional per month. Additional Services included:
- General Services
- Level 1 Primary Care
- Level 2 Extended Care
- 24 Hour Supervision
- Constant reality orientation and intervention with dementia, and confusion
- Overnight assistance and, bedtime help, and diaper changes, etc.
- Extended services provided by the Director of Nursing and the entire nursing staff.
Please Note: The Level of Care fee is in addition to the base monthly room rate.
(Level of Care services are not pro-choice. Level of Care services are based on the assessment. All assessments are done before a potential resident move in. Also, not everyone needs to be on a particular level of care. The assessment is based 100% on an individual's personal care needs.)
Other Information
Community Fee – Our community fee is $5,000.00 and it is due after the 4-week trial period (30days). Hayes Manor is 100% private pay. We do not perform any 3rd party billing.
Pictures of Available Units for Occupancy – We will send pictures upon request.
We do require medical information. We have a medical evaluation form (MDE), specifically for this matter in our detailed literature brochure. Ben, I have attached a copy of our brochure to this email as well.

Professional Care Staff
We have a professional care staff that ensures we are able to meet all residents' needs. From dressing to bathing, to assisting them to and from meals, to administering medications, we do our absolute best to provide the quality care that each and every resident deserves.
24-hour Nursing and Security
We have 24-hour nursing and 24-hour security, with video monitoring to all open and exit doors.
Staff Roles and Transition Assistance
All of our staff are assigned specific roles to assist with the transition into our community.
Resident Transportation
We utilize Para Transit to schedule residents' transportation to and from doctor visits. Family members are also welcome to take their loved ones to and from appointments as well.
Addressing Resident Concerns
The residents can come to our main office and report any questions or concerns they may have, and depending on the issue at hand, that respective department head will address the residents' concerns.
Dining Services
All of our meals take place in our Terrace Room Dining Hall, in which our Dietary Staff will serve the residents' meals restaurant style.
Admission Process
Upon admission, we get to know who the resident's emergency contact person, POA/Fiduciary, and loved ones or point of contact's information. Our nursing staff as well as our administrative office will keep everyone in the loop.
Respite Care Services
No, we do not offer Respite Care services.
Apartment Availability
All unoccupied apartments come 1st person 1st served. There is no waiting list.
Levels Of Care and Availability
Yes, our Levels Of Care is our age in place program, which is also specified in our "Welcome" email. No, we currently do not have a waiting list. All available units are on a 1st come first serve basis.

Our address is 2210 Belmont Avenue, Philadelphia PA 19131. It's just 2 Blocks from the Bala Cynwyd Shopping Center! Across the street from the Brand-New Philadelphia Police Barracks, and right next to The Bala Golf Club.General Information
Hayes Manor is both a Personal Care Home & Independent Living Facility. We start caring for seniors at age 65 and older. Our community has both private units with private bathrooms, and private units with shared bathrooms. The living units’ base monthly fee ranges from $2,000.00 - $2,600.00 per month. The price depends on the unit of choice. We have 3 meals per day (Breakfast, Lunch, & Dinner) and we offer additional 2 snacks daily.- Breakfast is from 8:00 a.m. - 9:00 a.m.
- Lunch is from 12:00 p.m. - 1:00 p.m.
- Dinner is from 5:00 p.m. - 6:00 p.m.
Additional Services
- 24-hour security monitored by a closed circuit security camera system
- 24-hour nursing staff with a nursing station and infirmary - The doctor comes on a weekly basis or by appointments.
- Daily housekeeping service – Our Housekeepers clean all rooms daily in personal care and weekly for independent units.
- Weekly laundry services – Laundry is washed, folded, and delivered to the residents’ units twice per week.
- Weekly shopping trips – We take our residents shopping to the Bala Cynwyd Shopping Center, Target, and other local stores. Residents join in on shopping but during Covid 19, we pick up their items for them.
- Barber/beautician services - Our Hair Stylists come on a bi-weekly basis.
- Daily Activities Program - We also have a wonderful activities program that provides daily entertainment, arts & crafts, music therapy, bingo, board games, trips to plays, museums, Atlantic City (depending on the weather) just to name a few.
- Ecumenical services in our chapel subsidized
