Hartwell Place Overview

Hartwell Place is the memory care assisted living community of Chicago Methodist Senior Services (CMSS) located at 5520 N. Paulina St., Chicago, IL 60640. It is an intimate 28-studio apartment building, with each suite having a full bathroom. We have a lounge on each floor and an activity room on the first floor, with a balcony overlooking a beautiful courtyard. Residents participate in a variety of activities, under the direction of Ana, the Community Life Coordinator. We incorporate scenic drives and outings, as well as art therapy, reminiscence, music, creative expression, and physical instruction into the daily schedule. Laundry is done in-house, meals are provided in our dining room and we have 24-hour nursing care.

Unique Features

Please make note that we are smaller than most memory care buildings, yet we, as well as families, have found that our environment is conducive for folks with memory care needs, and who have challenges with navigating space. Per the Rate and LOC (Level of Care) Schedule attached, room and board rates are dependent on the size of the room. The level of care is determined by a resident acuity assessment that assesses activities of daily living in areas of cognition and behaviors, mobility, personal care (bathing, dressing, bathroom), eating, medication, and monitoring, as well as coordination of appointments and services.

• When a suite is identified, a non-refundable $1000 room hold admission fee is submitted (refundable only if application is denied).
  *If wanting to transition into a move, respite stays of about a month are also available.
• After touring and deciding upon a suite, next is to complete an Applicant Financial Report and Residential Application before an in-person/virtual assessment is scheduled, for financial clearance.
