Prior to admission, a non-refundable community fee is required. This deposit covers the cost of application paperwork, medication set up, dietary set up, the cost of furniture, and other associated administrative fees.

Furniture:

The cost of furniture is included in the deposit. However, residents can supply their own furniture (except for the bed). There is a $200.00 fee if the mattress needs to be replaced at any time.

Levels of Care:

Base Level: $300.00
Resident requires minimal supervision with activities of daily living, safety oversight, and medication management.

Level One: $550.00
Resident requires supervision with activities of daily living, requires cueing or verbal reminders for toileting, minimal nursing care needed including daily vital sign management.

Level Two: $800.00
Resident requires light assistance with activities of daily living, verbal and physical assistance with toileting and general incontinence oversight 1-3 times weekly, light nursing care with advanced medication management (including but not limited to blood thinners, diabetes oversight, injections, etc.) and/or oxygen management.

Level Three: $1,050.00
Resident requires increased physical involvement with activities of daily living, hands-on assistance with meals, supervision with ambulation, requires moderate nursing care including wound care, and/or palliative care coordination, a bed alarm for safety, and/or occasional redirection of behaviors (3+ times weekly).

Level Four: $1,300.00
Resident requires extensive assistance with care, requires verbal and physical assistance with toileting and general incontinence care, oversight of catheters and/or ostomies, one-person transfers, and/or consistent redirection of behaviors (3 or more times daily).

Level Five: $1,550.00
Resident requires total assistance with care, totally dependent for all aspects of toileting, frequently requires one-person oversight for behaviors, special equipment for transfers, two-person transfer, and/or coordination with outside agencies during end of life care.

Pricing Information

Room Rates:

Private Room: $200.00 per night

Large Private Room: $230 per night

Semi-Private Room: $180.00 per night

Day Care: $85 a day

Additional Services:

Guest Meals: $5.00/meal

* $85 a day covers no more than 8 hours and two meals. If the resident stays longer than 8 hours there will be an additional charge of $5.00 an hour. With the exception that the resident cannot stay longer than 11 hours in a day.

Deposit:

If the respite individual is staying for more than five days, there is a $500.00 advance payment that is due prior to the respite stay.

Furniture:

There is a $200.00 fee if the mattress needs to be replaced at any time.

Resident requires assistance with medication management and light assistance with activities of daily living.

Level One: $16 per day/night
Resident requires reminders for toileting, monitoring blood pressure, oxygen levels, or blood glucose, and/or the resident uses an outside pharmacy that supplies medications in pill bottles.

Level Two: $25 per day/night
Resident requires verbal cues for care, supervision with toileting, advanced medication administration performed by a nurse (insulin management, crushing medications, injections), weekly redirection of behaviors (1-2 times weekly), and/or a therapeutic diet.

Level Three: $33 per day/night
Resident requires verbal cueing during meals, increased involvement with activities of daily living, daily ambulation with a one-person assist, and/or occasional redirection of behaviors (3+ times weekly).

Level Four: $41 per day/night
Resident requires hands-on assistance with meals, total assistance with care, oversight of catheters, ostomies, and feeding tubes, one-person transfers with/without wheelchair, and/or consistent redirection of behaviors (3 or more times daily).

Level Five: $50 per day/night
Resident requires special equipment for transfers, two-person transfer, and/or end of life care.

Care Staff Team

We have RN's, Medication Technicians, Licensed Nurses Aids, & Caregivers on the care staff team. The care staff are required to have dementia training on hire as well as annually. We have monthly in-service training by a Physician's Assistant. These trainings cover anything from hoyer/mechanical lifts, de-escalation, feed assist, and redirection. We also train if we see trends of topics that care staff want to learn more about.

Safety and Wellbeing Measures

We have several measures for safety and wellbeing. For safety: our building was developed with memory care in mind. The building is set up to ensure the physical safety by enacting locked exterior doors but allowing outdoor space for fresh air through our courtyard and back patios. We have only 1 floor to alleviate use of stairs. Our team is set up so there are eyes on every corner of the community. Administration in the front, nursing out on the floor with medication carts, activities on the back end, and HR on the opposite side. Residents who are cognitively able can use their call bell system to reach assistance. If residents are unable to use their call bell, we perform routine checks to ensure each resident is safe. Our employees undergo a criminal background check. We investigate all incident reports, and each one is escalated to the state of NH if it warrants a safety concern. Social Services Supports

We offer social services supports for residents and families while they transition into our community. We also perform check-ins with the resident during their transition to make sure they are comfortable and getting acclimated.

Transportation and Visiting Providers

We offer transportation for outside medical appointments at cost. We provide weekly outings as part of our activities program every Thursday. We do offer visiting providers such as primary care, geriatric psychiatry, podiatry, and hairdressing services in-house.

Dining Experience

Our dining experience is restaurant style. Residents are provided with the main meal or alternative and it changes daily. We have a hydration & snack pass twice a day. We can also provide therapeutic diets from a doctor's order.

Grievance Reporting

Residents may directly report issues to nursing or the director. Some residents also feel more comfortable telling a family member about a grievance and then we communicate with the family to resolve it. We also hold a monthly resident council meeting.

Care Assessments

Care assessments are completed every 6 months or during a significant change in condition in which the nursing team will communicate these changes and discuss a care plan moving forward. Families may also request care plan meetings at any time to go over goals of care.

Day Program and Respite Stays

We offer a day program which is an 8-hour-a-day program. The minimum is 2 days per week up to 5 days. This service includes all activities, 2 meals, and any care needed during the day. We offer respite stays from 2 weeks up to 30 days. Please see attached day program and respite stay pricing.

Pet Friendly Environment

While we do not allow residents to have pets, we are pet friendly. We often have office dogs in the building and families will frequently bring in dogs or cats. We also have a visiting pet therapy dog named Takoda.

Annual Flu Clinic

We have an annual flu clinic in October. We used to have a COVID clinic, however, there was a shortage of supply and there was a delay in receiving the COVID shots until January.

Age in Place Community

Harmony Homes by the Bay is our age-in-place community. We care for residents through total dependence of care. We also have added a new service called Unity Hospice that cares exclusively for residents of Harmony Homes.
