Our Commitment to Quality Care and Safety

We are staffed 24/7 with our Medical Director, RN's, LPN's, CNA's, Administration, Housekeeping, Dietary, and Maintenance Personnel. We are all mandated to successfully complete annual training that coincides with Federal and State regulations. We are required to perform competency tests that reflect our ability to provide proper care.

We have a Safety Committee that reviews and implements safety protocol to ensure everyone is safe within the facility.

Our Discharge Planning Team provides resources including follow-up care within the home for our patients returning to the community.

Yes, we provide transportation to all our resident's appointments relatable to their Level of Care.

Dining and Nutrition

We have a restaurant-style dining experience where patients may select from our alternative menu to suit their preferences (within their ordered diet). Encouraging independence throughout our facility especially during meal times can promote quality of life. We also encourage guests to enjoy meals with their loved ones at the facility.

Communication and Feedback

We have a grievance process that family/residents are educated upon at Admission. A grievance form is provided and explained on how to initiate a complaint. All the steps are reviewed and reassurance is covered as this form "Helps us To....Serve you Better."

Our Nursing and Social Services Team members both provide updates upon any change for our patients, whether it be room changes, medication changes, behavioral etc.

Additional Services

Currently, we are not accepting Respite Stay but if/when we start that up again it will be our Private Pay rates which are reflected in the prior attachment.

Yes, we have a Pet Policy that requires updated vaccination status by each pet's veterinarian. We also have weekly Pet therapy visits to the facility. Pre-approved Canines aka "Pet Therapy Dogs" visit our facility once a week for hugs and kisses. Those residents who had/have pets at home really enjoy this type of therapy.

Yes, we do have a waitlist for SNF and ICF clientele. Anyone interested would have their PCP fax us clinical information (Diagnosis, office visit, medications) to start this process.

We offer vaccinations within our facility as a convenience to our residents. If preferred, our residents have the right to seek alternative methods of vaccinations.

Specialization in Geriatrics

Our facility specializes in Geriatrics where our entire focus is on the Aging Process and how to provide the highest quality of care for our residents. Our Team is educated on a regular basis on the sensitivity of the elderly population and their needs. This training includes in-depth Trauma care that reaches to the core of our patients and their past history. Each one is unique and their story impacts the care they need.
