Do you guys currently have a waitlist?
Currently we do not.
Could you share pricing and any other general information?
Private pay would be $335.50/day, this covers room and board. There a few things that are not covered under this price such as haircuts and vending machine usage.
Could you provide an overview of the types of professionals on your staff and the training they undergo at your facility?
We have multiple professionals in the building including RNs, LPNs, CNAs, physical therapists, occupational therapists, licensed social workers, etc. These professionals have licenses prior to being hired. We also do annual training for all staff. These trainings include communication, dementia, and facility policy among others.
Could you tell me about the measures you have in place to ensure the safety and well-being of your residents?
There are multiple measures put in place, it truly depends on which aspect you are speaking of. For our fall risk residents we have low beds and mats, for our residents who may wander, we have a wanderguard system in place, For overall safety, we have nightly locked doors and staff located near all exit doors during the day.
Do you offer any resources or support services to aid residents' transition into the community?
We do, our social services director helps with getting community resources for all residents before they leave the facility. This could include home health nurses, home wound care, community dialysis center set up, or referrals sent to assisted living. She is also able to get any medical equipment needed in the home upon discharge.
Do you offer transportation services for residents?
Yes, we offer transportation via wheelchair van. If a resident does need stretcher transport we are contracted with transport companies to accommodate those residents as well.
Could you tell me a little about your dining experience?
Residents are free to eat in the dining room or in their room. We have a set menu for the month and a member from our dietary department goes over the menu weekly with each resident to pick out their choices. We also offer alternatives such as cheeseburgers, salads, and sandwiches in case they don't enjoy the main meal choice. There are snacks kept on the units at all times. We are also able to accommodate most diet orders such as puree, thicken liquids, and minced.
What is the process that takes place if a resident has a complaint or an issue they want addressed?
The resident can tell any staff member or fill out a grievance form. This will be reviewed by our administrators and social worker. From there a plan will be put into place to ensure the correct changes can occur.
What is your policy on communication with resident's families? How do you keep them updated about their loved ones' well-being?
We communicate with families listed on the residents chart. We are unable to give any information to any person calling or stopping in that is not listed in the chart due to privacy policies. If an incident or change in status does occur, the family is contacted by the RN Nursing supervisor on shift immediately. If there are no changes and you would just like an update you are free to call in and we can get you to your loved ones nurse so they can give you an in-depth update.
Do you all offer short-term or respite stays? How much does this cost?
We offer respite, long term care and short term stays. All stays cost the same $335.50 / day if paying out of pocket. We also participate with many insurance providers as well as Medicare and Medicaid.
Are you all a pet-friendly community?
We are! We welcome pets, as long as they have all of their vaccines and are under the owners control (not jumping on other residents, etc. )
Are residents able to get Covid vaccination and flu vaccinations in the community?
They are able to go out to the community to get these, however we do offer them in house. Our Infection control nurse is able to administer these vaccines right in the comfort of their room.
Are residents able to age in place?
We are a nursing home, we are also contracted with many hospice providers in the area. Currently we do not.
