Room Availability

Currently, I have shared rooms available. This is our companion suite and is $3,040/month. I will have a Royal Suite available early next week. The cost for that room is $4,408/month.

Specialized care is available when needs are identified through the Resident Assessment done at admission and on a quarterly basis. Additional fees may be added, depending on the level of assistance needed.

Nursing Staff Department

Our nursing staff department consists of caregivers whom we refer to as Resident Service Staff (RSS). Some of the RSS’s have their CNA certificates, but it is not required. We provide on-the-job training at the time of hire. New staff are paired with seasoned RSSs for several shifts to shadow and complete return demonstrations of the care they will provide. This includes activities of daily living such as bathing, bathroom assistance, dressing, incontinence care, feeding residents, transferring residents, skin care, oral hygiene, oxygen therapy, and catheter care.

Medication Administration

Our med room consists of LPNs and Medication Assistants who have been trained to pass medication. All Medication Assistants have completed the Department of Human Services approved Medication Administration training course and passed the required test with a score of 90% or higher. They must also pass four medication administration observations conducted by me, as well as four MAR reviews and two Medication administration observations annually. They receive diabetic training from a certified diabetic trainer and must be recertified annually.

Licensed Practical Nurse (LPN) Availability

I have my LPN license and am available 24 hours a day for any questions they may have.

Dietary Staff

Our dietary staff consists of Cooks, Dietary Aids, and Dishwashers. Several of our cooks and our dietary director have their Serve Safe certificates. New dietary staff are trained by seasoned dietary staff.

Staff Training

Upon hire, all staff receive training on resident abuse, resident rights, Fire Safety, Eating/Drinking/Toileting/safe transferring, Recognizing Residents as Individuals, Observing and reporting changes in residents' condition, Infection Control, and approach to care. Caregivers receive this and additional training as well as the Department of Human Services caregiver training, which ends with a test to receive their caregiver certificate. All facility staff receive a minimum of 12 hours of annual training covering infection control, dementia training, nutrition, safe transfers and fall prevention, residents' rights and abuse, Blood Borne Pathogens, Fire Safety and Evacuation, Emergency Preparedness, etc.

Wander Guard System

We do not have a locked unit; however, we have a wander guard system in place for residents who may be at risk of elopement. Our exterior doors are locked at 11 pm and unlocked at 6 am. We have measures for safety with medication administration. Residents leaving the facility for an appointment or with family are asked to sign out and back in so that we are aware of who is in the building or out at all times.

Therapy Services

We bring in Physical Therapy, Occupational Therapy, Speech therapy, Home Health, and Hospice support as needed. Our staff work well with new residents to help with transition into personal care. Our Activities Director excels at encouraging all residents, especially new ones, to participate in activities to feel included.

Resident Meetings and Preferences

Our Activities Director, Maintenance Coordinator, Housekeeper, Wellness Coordinator, and Dietary Director meet with new residents individually to review needs and preferences and answer any questions the resident may have.

Transportation and Medical Services

We do not provide transportation for residents; however, our nurses and MA's can schedule the Blair Senior Service Van for transportation. The cost is $2.00 each way. We also have a physician, Dr. Rifkah, who visits the facility so residents do not need to leave for primary care physician services. New residents can choose to switch to him. We have a lab that visits weekly for blood draws, and we can arrange for x-rays on-site.

Dining Services

We have a beautiful dining room for our residents. They have a menu, and the dietary staff inform them of their lunch and dinner options. If a resident dislikes the menu, alternatives are available such as cheeseburgers, pizza, soup and sandwich, peanut butter and jelly, or a salad. We accommodate special diets, mechanical grinds, pureed, thickened liquids, and cut-ups. Most food is homemade rather than frozen and reheated. Families are welcome to enjoy a meal with their family member with a two-hour notice. The cost for a family meal is approximately $8.

Communication and Meetings

Families and residents are encouraged to communicate concerns to me or the department manager. Our managers are easily accessible and have an open-door policy. Managers meet quarterly for quality management review, and daily communication is possible given our facility size. Monthly resident council meetings are held for residents and department managers.

Resident Safety

If a resident falls or requires ER attention, we will contact the resident's POA. Families can call the facility 24 hours a day to check on a resident. Annual family meetings review changes and provide updates from the year.

Pets and Community Environment

We have a community cat that resides here. Residents can bring small dogs, cats, or birds provided they can care for them. Families are welcome to bring in their dogs for visits, provided we have a copy of the dog's vaccination record.

Vaccinations and Goal

We provide flu and pneumonia vaccines annually. COVID vaccines are being considered for annual offerings. Our goal is to maintain residents here and allow them to age in place, providing assistance as needed. We are successful in this, with residents ranging from independent to needing extensive assistance. Our oldest resident is 104. Hospice, PT, OT, Home Health, etc., can be brought in to support aging in place.
