Staffing
We are staffed in our Assisted Living and Care Center only. Independent Living apartments are not staffed.
Assisted Living
Our staff consists of Home Health Aides, who are here 24 hours a day. Nurses are available during business hours with an on-call service for emergencies that may develop after hours. Our residents have the option to be followed by our in-house Doctor or Nurse Practitioner, or continue seeing their own doctor outside of our community. We have an RN Clinical Administrator who oversees all resident care and supervises the other nurses and aides.
Care Center (Skilled Nursing/Long Term Care/Nursing Home)
We have nurses and resident assistants in the building at all times. Clinical Coordinators (Nurse Managers during business hours) oversee all healthcare services for our residents and report to our Director of Nursing, who is available during business hours. We also have an in-house Doctor and Nurse Practitioner.
Trainings
All staff have annual trainings, and additional trainings depend on their certification. We communicate regularly as a team, meeting in person daily and updating our directors at our corporate office.
Safety
The safety of our residents is of utmost importance to us. Our building entrances are locked at all times. Only those with special access (key fob) or those allowed inside by our receptionists can enter. For residents considered to be an elopement risk, special assessments are completed, a special order obtained from a physician, and when appropriate, we offer placement in one of two controlled-access memory care units.
Resources
Our team stays up to date on current resources available in Isanti County. We keep resources on hand and provide them to anyone who asks. In our Care Center and Assisted Living buildings, we have Resident Services staff who help with discharge planning as needed and know how to submit special referrals or involve county workers and other representatives.
Transportation
We have a bus that can transport residents around the Cambridge area during business hours.
Dining
We offer various dining options. We have a wonderful culinary team providing delicious food. We have an on-site Restaurant, Bistro, and dedicated dining rooms for Memory Care Assisted Living and Care Center units. Food is prepared in the main kitchen, with a variety of options always available. Breakfast can be ordered anytime of day, which is very popular with our residents.
Independent Living
Each building has a Housing Director who addresses any concerns from residents or families.
Assisted Living and Care Center
Our Resident Services Director and Household Coordinators address concerns from residents or families.
Costs
Costs vary significantly based on the type of living and layout. We can share specific prices once you've narrowed down the type of living you're interested in.
Responsible Party
Residents usually appoint a "Responsible Party" or Primary Contact for regular communication. In our care settings, we schedule Care Conference meetings where families and the resident can attend in person for updates, questions, and concerns.
Short-Term Rehab and Respite
We operate a Short-Term Rehab in our Care Center. Respite care is available in our Care Center, subject to bed availability. All referrals go directly to our Admissions Nurse for review. The State of MN determines the daily rate for any stays in the Care Center based on assessments, so the daily cost varies for each individual.
Pet Policy
We are pet friendly. Our pet policy allows pets in our Independent and Assisted Living apartments. We require a deposit, vaccination records, a weight limit of 20 lbs or less, and a limit of two pets per apartment. Residents must be able to care for their pets independently without staff assistance. Special pet visitors are always welcome, even if they cannot live here.

We currently have a wait list, which depends on the type of living you are interested in.

Residents can request vaccinations here.
Levels of Care
We offer a full continuum of care with several separate levels. This means you may start in one type of living and later move to another apartment or room for a different level of care. For example, someone may begin in an independent apartment but later move to Assisted Living to receive more structured support.
Independent Living
I hope it's ok with you, but I am going to send over some information on both Independent and Assisted Living. I don't want to risk not getting the info to you, as I am here only until 4:30 today and not back in office until Monday morning. I want to be sure you have what you need before the weekend.
So, to get us started, this first email has only Independent Living options attached.
The Terrace
Our Terrace is an Independent Apartment building, complete with 50 apartments. Laundry rooms are conveniently located inside each apartment. A special continental breakfast is available to these residents only, served in our Restaurant. There is a special underground, heated, parking garage that Terrace residents may rent spaces in. This building is our newest, as it opened in 2018. Because it is located directly on the main part of Campus, all amenities are very convenient to access.
Mill Ridge
Mill Ridge is our original Independent Apartment Building, located just across the river from the main part of campus. We have 56 apartments inside. This building has had many beautiful renovations done over the last few years. As you can see, prices are a bit lower. Laundry rooms are located on each floor, vs. inside the apartments. All residents in this building still have access to Campus amenities, they would just either drive over or ride the GracePointe bus. There are detached garages available to rent.
The Village Townhomes
Our Village Townhomes consist of 40 homes. They surround the parking lot around the main part of campus. Prices are great, some utilities included, but not all. Each townhome has an attached garage and small driveway. Each also has their own patio. These too have been recently renovated.
In any Independent Living option, there are no staff but residents all have a right to hire their own services from an outside agency. There are many structured activities, fun planned outings, special celebrations and treats offered in our Independent Living. The anticipated wait time for our IL apartments is likely to be around 6 months. Our townhomes wait list is quite extensive and we do anticipate longer than 4-year wait for those that do not live on Campus.
Assisted Living
Next up would be Assisted Living!
Commons
Our traditional assisted living apartments are in our Commons. This option is private pay only. Monthly rent also covers utilities (basic cable, landline and public wi-fi), two restaurant meals daily, two snacks daily, and weekly housekeeping services. Care packages are customized to fit each individual's needs and begin at $420/mth. Again, we have nurses here during business hours and home health aides here 24 hours a day. Options to have primary care right on site as well as some specialty services.
Commons Suites
Our Commons Suites are designed and look exactly like our Care Center rooms do in our Skilled Nursing/Long Term Care. They come furnished, three meals per day and two snacks are provided, utilities and weekly housekeeping as well. Care Packages are also customizable. This is our unit designated to accept Elderly Waiver under Medicaid.
Arbor
Last but not least, our Arbor. Arbor is our Assisted Living Memory Care Unit. It is designed to look very similarly to our Commons. The unit consists of studio and 1bd apartments. They have their own private dining room and outdoor patio. This unit has controlled access for safety. Three meals, two snacks, utilities, up to 3 loads of laundry per week and weekly housekeeping is included. Care Package information is indicated inside the brochure but is broken down into one of three different levels.
Activities and socialization are other huge pieces of Assisted Living life. Our Activities team always goes above and beyond to plan special events for our residents. Families are always welcome to attend with their loved ones as well.
