Waitlist and Availability
On assisted living, we have a waitlist for the larger 1 bedroom and 1 bedroom with den apartments. However, we currently have ready-to-move-in studios in two sizes available.
Pricing and General Information
We have attached our rate sheet and handouts for levels of care to provide detailed pricing and general information about our services.
Professional Staff and Training
Our campus is a full continuum of care facility. In the IL/ALF, we have QMAPs, PCPs, activities assistants, a Director, and a nurse. Additionally, we have a full dietary team, social worker, activities director, maintenance director, transportation driver, housekeeping team, and access to our PT/OT/SLP team with insurance approval. We also have arrangements with Summit Medical Group for onsite medical care. All staff receive mandated education training from the state level, including fire safety, resident rights, infection control, and abuse prevention. We are also working with Dementia Together for education on their Specl program for care specific to those with cognitive impairment.
Safety and Well-being Measures
Our residents have access to a call system in both the main apartment space and the bathroom, along with a pendant system for emergency care. Meal arrangements and a check-out clipboard help monitor resident well-being. Our main entrance is open during work hours, with after-hours access controlled by a rehab nurse or issued fob devices. All external areas are monitored by cameras, and alarms alert staff if a resident exits after hours without a fob or staff assistance.
Transition Support Services
We work with moving companies to aid in the moving process and have designated residents who encourage new residents by explaining activities, schedules, and encouraging socialization. We provide oversight for the first 2 weeks to ensure new residents are comfortable with their new environment.
Transportation Services
Transportation is available for Greeley medical appointments, with case-by-case arrangements outside of Greeley. We also organize several outings a month, such as shopping trips and lunch groups, with residents signing up at the front desk.
Dining Experience
Each area of our campus has its own dining room, with the IL/ALF being the largest. Residents have multiple seating options and meal choices, including a chef’s choice meal and a substitution menu with about 30 options available at all meals.
Complaint Resolution Process
We have a Grievance policy that involves forms, social worker and director involvement, and a committee review. Information on contacting the OMBUDSMAN is posted and available upon request.
Communication with Families
Designated contacts are notified of any hospital transfers, falls, or other concerns. Care conferences are held in compliance with regulations and can be requested by families at any time.
Short-term or Respite Stays
We provide short-term respite stays with a daily rate dependent on the length of stay. The current rate is $200 a day, with price breaks for longer stays.
Pet Policy
Our ALF/IL allows for small pets, which must be managed solely by the resident. There is a pet fee.
Vaccination Services
We hold regular shot clinics for Covid and flu vaccinations and can assist in scheduling pharmacy visits for vaccinations to be done in the community.
Aging in Place
While we strive to allow residents to age in place, there are times when an individual’s care needs exceed the ALF level of care. We have multiple levels of care within our building to accommodate residents as their needs change.
