Overview of Staff and Training

We have a diverse team of professionals, including: physical therapists, occupational therapists, speech therapists, physicians, nurse practitioners, and specialists in vision, podiatry, hearing, and dietetics. Each professional undergoes comprehensive training at our facility to ensure the highest quality of care.

Safety and Well-Being Measures

To ensure the safety and well-being of our residents, we utilize wander guards for those at risk of elopement and have secured, locked doors for the entrance and each resident's floor.

Support for Transition into the Community

We have two social workers specializing in TCU and LTC who collaborate with residents, their physicians/nurse practitioners, therapy teams, and business staff to create safe and feasible discharge plans for a smooth transition back into the community.

Transportation Services

Transportation services depend on the type of insurance held by the resident. Our HIM schedules all resident appointments and arranges rides to and from these appointments.

Dining Experience

Residents are provided with three nutritious meals a day, featuring seasonal rotating menus tailored to meet each resident’s dietary needs. We also prepare specialty meals for our large Southeast Asian population.

Complaint and Issue Resolution

When a complaint or issue is raised, the recipient fills out a grievance paperwork, which is then passed to the corresponding social worker and administrator. An investigation is conducted, and appropriate interventions are implemented to resolve the issue.

Communication with Families

For residents with a guardian or DPOA, we inform them of significant changes, appointments, and concerns. Independent residents can designate someone to be informed on their behalf.

Short-Term or Respite Stays

We offer short-term stays in our TCU. The daily rate is $438.13, which may vary depending on insurance. Our social services team can assist in finding home health or respite services as the resident approaches discharge.

Pet Policy

Pets are allowed to visit, but they cannot reside in the community.

Vaccinations

Residents are able to receive COVID-19 and flu vaccinations within the community.

Aging in Place

Residents can transition from the transition unit care (TCU) to our long-term care floors if they are unable to return home or to the community. We have room available for such transitions.
