Our Staff

We have on staff RN, LPN, CNA, social workers, PT/OT/ST. All staff undergo orientation and ongoing inservice training for the facility.

Safety Protocols

The facility has specific protocols for safety and natural disasters in place that staff are trained for.

Discharge Planning

Discharge Planning is taken care of by an LPN and social worker to provide community transitions for residents who wish to transition to the community.

Transportation

Transportation is not provided by the facility. We work through residents' insurance to set up needed transportation to/from appointments and ask that family accompany them.

Dining

Gallatin Center has a main dining room that is open for breakfast, lunch, and dinner; residents have the option to receive meals in their room. A select menu is available as well as a 24-hour kitchen with an ala carte menu.

Private Pay Rates

Private room- $290/day, semi-private room- $270/day, private skilled- $450/day.

Communication

Nursing communicates changes in care to families as they arise. Social services hold care plan meetings periodically for families to meet with the interdisciplinary team for questions/concerns.

Respite Stays

Short term/respite stays are available and at the same cost as the daily rates above.

Pet Visits

Pet visits are allowed following vaccination records provided to the Activities Director.

Vaccinations

Vaccinations are available to residents in the facility.

Care Options

Short term rehab, long term care, and hospice care are available in the facility to allow residents to be cared for through any stage of care needed.

We can add your loved one to the waitlist if you choose by providing name, DOB, insurance, and contact information.
