Overview on the Types of Professionals on Our Staff and Their Training

We have Home Health Aides, Certified Nursing Assistants, and Trained Medical Aides who perform the day-to-day tasks, one on each shift.

Measures to Ensure the Safety and Well-Being of Residents

We offer various activities and have on-site maintenance personnel. For residents who tend to wander, they would need to be on the nursing home side as we have a wander guard system there. In assisted living, we strive to provide an environment that is as close to home as possible, so residents are free to come and go as they desire.

Support Services for Transitioning Residents

We understand that transitioning to a new environment can be challenging, so we take the time to “get to know” each resident. We provide reminders when needed, re-direction, and regular assessments to ensure that current services meet their needs.

Transportation Services

We do not offer transportation services directly, but we can assist with arranging transportation. Providers from Sanford and Essentia clinics perform rounds on-site a couple of times a month and as needed.

Dining Experience

In Frazee, we are attached to the nursing home, so food is prepared in their kitchen and then brought over to assisted living in a steam table to be served in the communal dining room. In Vergas, food is prepared by the on-duty staff.

Complaint and Issue Resolution Process

We have a grievance policy and form that hangs in the dining room. Residents can bring concerns to the staff using that form or verbally. They can also contact the state ombudsman’s office if they don’t receive a satisfactory resolution. When using the form, it is sent to management, and a meeting is held to ensure the resident feels heard, and they work together for a resolution.

Policy on Communication with Residents' Families

We can update family members as much as they desire. All changes in condition, such as illness or falls, are promptly conveyed.

Short-Term or Respite Stays

We do not offer short-term or respite stays.

Pet Policy

We review this on a case-by-case basis.

Covid and Flu Vaccinations

Yes, we are able to obtain all vaccines from our local pharmacy. We use Minnesota Immunization Information Connection, a statewide site, to register vaccines given and to obtain history.

Aging in Place

Assisted Living is only for those that are independent in their activities of daily living. In Frazee, a resident can move to the nursing home side if their needs are unable to be met by our Assisted Living staff. We are currently full but are building our waitlist.
